Position Summary:
Responsible for development of nursing care plans and counselling for patients with diabetes and/or co-morbidities such as renal, cardiovascular disease and mental illness
Responsible and accountable for decisions and actions in the provision and management of nursing care for patients with diabetes in a collaborative inter-professional team environment
Broad understanding of resources available in the community to help support caseload of patients through critical thinking and problem solving to ensure best practices are maintained
Assist and support clients to gain the knowledge, skills, attitude and self-awareness necessary to effectively manage his/her diabetes
Monthly rotations in the inpatient units at the Mississauga Hospital to support bedside teaching for diabetes and safe discharge.
Function as a part of a multidisciplinary team, and act as a resource for other health care providers and the community
Using a variety of teaching methods, incorporating the principles of adult education and a variety of client engagement or empowerment modalities to facilitate behaviour change (ie motivational interviewing, stages of change )
Facilitates group education programs for patients as needed
Develops appropriate education materials to meet the needs of the clients
Assists in program planning and evaluation
Key Qualifications:
Certified Diabetes Educator
Membership in the Diabetes Educator Section of Diabetes Canada preferred
Minimum 5 years' experience in counselling clients with diabetes on an individual and group basis
Good working knowledge of diabetes and comorbidities through the lifecycle, adult teaching principles and patient engagement strategies
Good working knowledge of diabetes medications, including insulin
Highly effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written
Proficiency in computers, Microsoft Office and experience with electronic medical records and good keyboarding skills are essential
Reliable, flexible, creative professional, able to work effectively and collaboratively in a multidisciplinary team approach required
Experience using medical directives an asset
Additional language an asset
Positive attendance and work record essential
Member in good standing with College of Nurses of Ontario
