Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Registered Nurse Weekend Coordinator to join our Palliative Home Care team to provide professional support services in the homes of clients of all ages who have been diagnosed with a life limiting illness. Please note that the primary location will be based out of Seventh Street Plaza and may be required to assist at other sites in the Edmonton Zone. This position requires a 11.08 hours' shift every Friday, Saturday and Sunday according to FTE. As a self-directed team member with Continuing Care in the Edmonton zone, the Registered Nurse Weekend Coordinator works collaboratively with the health care team and has responsibilities, including assessment of needs, delivering appropriate therapeutic nursing services within professional practice standards, coordinating resources, establishing goals for client delivery, and evaluating outcomes. This position also entails program triaging during the day and will take a lead role in planning the workload for the weekends; includes liaising with Community Care Access to provide direct care in the community.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:
Minimum of 3-5 years of recent Continuing Care nursing experience. Must be able to work independently and within a team environment. Self-directed and the ability to work independently & use sound professional judgement. Excellent organizational skills to be able to prioritize and efficiently organize many details in a timely manner. Communication and assessment skills to work cooperatively & effectively with the healthcare team members, clients, & their families in a fast-paced environment, identifying needs via tele triage & resolving conflicts. Must be able to cope with high levels of stress, fluctuating priorities, and time sensitive deadlines. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet. Physically capable of performing nursing tasks which could include transfer of clients. The successful candidate will work independently in clients' homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. Preferred Qualifications:
Baccalaureate Degree in Nursing. Palliative care experience is an asset. Demonstrated leadership and/or management experience. Ability to identify & manage risk and to mitigate and diffuse those situations. Triage experience. Experience with Meditech and Connect Care. 2 years current Case Management Experience in Continuing Care. Experience with introducing new initiatives and working on change management.
