Job Description

Sanguen Health Centre currently has two registered Nurse positions available.



One role will include operating the Hep C/HIV clinic with the Infectious Disease Physician; the other role is supporting Sanguen's Primary Care Outreach team at locations across the Region of Waterloo. There is a total of 1.8 FTE available, and the FTE for each role will be determined based on the successful candidates preference and availability.

Sanguen Health Centre has two positions open for Registered Nurses who are interested in collaborating with physicians, nurse practitioners, social support and outreach workers from across Sanguen Health Centre's programs and other health agencies to support those who may experience Homelessness, mental health or addictions.

Responsibilities



Works as part of the community based, interdisciplinary primary health care team providing leadership and coordination in client care Demonstrates safe, quality practice including Hepatitis C and HIV screening, prevention, and supporting treatment care plan of prescribers; Works at the top of scope of RN practice, including but not limited to providing safe administration of medications and immunizations, wound care, venipuncture, assessment and triage for emerging health concerns,disinfection/sterilization of equipment and environment, client education and assisting practitioners as requested. Assisting with developing care plans, care coordination across an interdisciplinary team and external referrals for clients. Support and collaboration with infectious disease physicians and the Sanguen team as appropriate. Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate documentation, participating in chart reviews, case conferences and case analysis. Maintains smooth and efficient flow of people through their visits. Practice from a trauma-informed, harm reduction, anti-oppression lens. Supports system navigation, referrals to and liaising with healthcare services Collaborates and supports the work of partner agencies who offer primary care, addictions, mental health, harm reduction supports; Other related duties as assigned. Driving the Clinic Bus, as required.

Qualifications



Minimum of 3 years' nursing experience - in a community setting, or combination of community and hospital or public health settings. Familiarity with infectious diseases including Hepatitis C and other sexually transmitted and bloodborne infections. Current certification as a Registered Nurse in the Province of Ontario with CNO membership. Demonstrated experience working with people who use drugs and people who are vulnerable, living in poverty, and/or experiencing homelessness. Must be comfortable being around, discussing and working with people who use or have used drugs and compassionate about the barriers faced by people who use drugs. Thorough knowledge and proficiency in current nursing principles and practices especially regarding Mental Health and Addictions. Ability to represent Sanguen's values while maintaining positive community relationships. Understanding of Anti-oppression principles and ability to work effectively with people of various cultural, age, and economic backgrounds. Ability to work effectively within a multidisciplinary team. Ability to work a schedule that may include days and evenings. Computer proficiency with electronic charting. Experience in primary care within a street/outreach nursing setting is an asset. Experience working with racialized populations, Indigenous peoples, women who engage in sex work, prisoners/ex-prisoners, people with mental health challenges, and people who have experienced trauma, is an asset. De-escalation and conflict resolution skills preferred. Is willing to be trained in driving the clinic bus

Hours and Location of Work



Hours will include daytime, evening, and weekend shifts. Number of hours per week will be determined based on the applicants preferences and availability

Providing services in multiple locations, including: 29 Young St (Waterloo), 130 Victoria St (Kitchener), and outreach in the Region of Waterloo. Outreach may occur on Sanguen's Primary Care Bus or van or at clinic locations in partner agencies such as Drop In or Shelter locations.

The successful candidate must be able to travel to and between these locations by personal vehicle. Mileage will be reimbursed for travel between work locations within a shift.

Compensation and Other Perks



The compensation for this role will be between $74,623.50-$95,800/year. 4 weeks' vacation which accrues biweekly, prorated to FTE. A comprehensive Employee Benefits package upon successful completion of the probationary period (if applicable). 15 paid sick days per calendar year.
