Your Opportunity:The Inpatient Transition Services (TSC) Coordinator works within a multi-disciplinary team in acute care and is responsible for appropriate referral to community programs/services, appropriate documentation, participation in conferences and team meetings, process, and outcome evaluation and professional development activities. Major responsibilities include: completing comprehensive electronic needs assessments, determining urgency/priority, developing and implementing appropriate service plans, coordinating service delivery across programs/settings, liaising with Continuing Care staff and other community services/resources, providing information and support to clients, families, and caregivers. The TSC provides ongoing guidance and mentoring to acute care unit staff to ensure the homecare referral processes are followed and ensures order are appropriately written for successful community care. The TSC provides clients and families with information and counseling with respect to care options and facilitates access to these. The TSC develops and maintains relationships with Home Care Case Managers, Integrated and Supportive and Facility Living Case Managers, Facility leadership, and other agencies. The TSC manages a caseload of clients while on the acute care unit who are current home care clients, new admissions to home care, clients needing complex discharge planning, and assesses clients for alternate levels of care. This is a fast-paced environment that requires a high degree of problem solving and organization. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
