The Transition Coordinator (TC) is a clinical consult-based position. TCS assesses patients' needs in the context of the whole person, including their life situation and determinants of health. Critical thinking and accurate interpretation of complex information from a variety of sources are essential to the clinical decision-making required for the determination of appropriate continuing care living options. The role of the TC is highly complex and includes comprehensive assessment, planning and coordination of care, as well as ongoing communication with other healthcare providers across the continuum of care. Major responsibilities include: completing comprehensive electronic needs assessments, determining urgency/priority, developing and implementing appropriate service plans, coordinating service delivery across programs/settings, liaising with Continuing Care staff and other community resources, and providing information and support to clients, families, and caregivers. The TC works within a multi-disciplinary team of health professionals and is responsible for appropriate referral to other programs/services, appropriate documentation, participation in conferences and team meetings, process and outcome evaluation and professional development activities. This position will also provide relief to Suburban Hospital sites across the Edmonton Zone. Applicants should be aware that there are physical demands. The successful candidate will work independently within acute care settings, auxiliary hospitals, mental health facilities, and clients' homes. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and assess tools, which weigh approximately 15-25 lbs. Work tasks and charting is computer-based, so there are extensive repetitive motion tasks.
