Carefirst Seniors & Community Services Association

is a non-profit community services agency. Since 1976, we have provided community support, home and personal health care services in a holistic manner to seniors, individuals with disabilities, and those with medical needs in the Greater Toronto, York, and Mississauga regions.



With a staff of 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including older adults, seniors and the physically challenged; around 20% of whom are "home-bound" and frail.



We are currently seeking a

Registered Nurse

to join our Diabetes Education Program.

Responsibilities:



Plan, deliver and evaluate diabetes education programs, such as group teaching, individual counseling, support groups and other initiatives to support people with diabetes Initiate and promote community diabetes education Practice in accordance with self-management model to assist patients with diabetes to achieve their optimum level of health and quality of life Participate in outreach initiatives to reach complex and marginalized populations Work with the team to plan, develop, implement and evaluate performance improvement plan Accurately complete documentation in a timely manner using EMR while maintaining confidentiality Provide clinical support for patients and physicians, perform follow up actions if required Collect daily statistics and compile timely reports to ensure continuity of care Perform any other DEP related tasks as assigned

Requirements:



Bachelor of Science in Nursing from an accredited institution Current registration as Registered Nurse with the College of Nurses of Ontario in good standing A valid Basic Cardiac Life Support (BLS) certification Prior specialization in diabetes or Certified Diabetes Educator or willing to work towards Minimum of 3 years of clinical nursing experience in a community-based healthcare setting Experience in developing, implementing, monitoring and evaluating community and health education programs Experience in educating individuals and groups by applying various adult learning principles Strong interpersonal skills and verbal and written communication skills to deliver information and work effectively with patients, families, and an inter-disciplinary team Demonstrates empathy and sensitivity to patient needs in a culturally diverse practice population Well organized and ability to multitask in a fast-paced environment Ability to exercise sound clinical judgment and to function in an independent, self-directed manner Proficient computer skills in Microsoft Office and medical EMR software (Telus Practice Solutions) Proficiency in English; and a second language reflecting the community served, such as Cantonese and/or Mandarin, is an asset A valid vulnerable sector police record check completed within one year
