The Registered Nurse works in collaboration with physicians and other interdisciplinary health professionals as a member of the Family Health Team. As a member of the team, the Registered Nurse is responsible for delivering quality nursing care to patients with a focus on assessment, health education, disease prevention and as well as early intervention and support for chronic health conditions. RN participated in a team-centered care in the community within the facility and in the promotion of continuing, family-centered care in the community.
QUALIFICATIONS
Certificate of Competence and current membership with the College of Nurses of Ontario
Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred
Current membership with RNO and evidence of liability insurance
Current Ontario Driver's Licence preferred
Demonstrated experience in Community and/or Primary Care Setting
Satisfactory Criminal Record and Vulnerable Person check

POSITION RESPONSIBILITIES

PATIENT CARE
Maintains CNO Standards of Practice
Functions within guidelines of clinic policy and procedure
Maintains complete and accurate medical records and answers patient inquiries in order to direct them to appropriate and comprehensive treatment under the direction of a physician
Preserves confidentiality of all patients with the FHT
Promotes an interdisciplinary approach to patient centered care
Communicates effectively with all providers of care. Communicates pertinent information accurate and promptly to the physicians and allied professionals
Utilizes the Nursing Process to assess, plan, implement and evaluate patient care in consultation with the physician, on an ongoing basis
Administers medication topically, through natural body orifices, through tubes by subcutaneous and intermuscular injection through IV access; as specified by College of Nurses of Ontario guidelines and FHT policy; records accordingly in chart
Change dressings, remove sutures and staples, remove catheters and change colostomy appliances when requested by physician
Maintains competence in all clinical skills pertaining to his/her area of work. Maintains ongoing awareness of change in technology, treatment of illness and nursing practices
Promotes and facilitates continuing competence in skills and ongoing education for self and all staff, to endure delivery of the highest standard of care
Facilitates patient/family education. Advocates for and practice "health and wellness" care and education
Assist primary care team (including assessment, triage, treatment, and follow up). Perform clinical tasks by conducting nursing assessments, health education/counseling, history taking, performing specific procedures according to the College of Nurses of Ontario standards of nursing practice or medical directives and providing follow-up care
Utilizing delegated medical acts, Registered Nurse will perform screening, physical assessments, vital signs monitoring, collect data and analysis, provide education, awareness and required referral to patients for harm reduction, patient wellness and signs and symptoms of chronic disease progression
Participates and provides support in chronic disease, including but not limited to smoking cessation, diabetes prevention and hypertension programs and service coordination programming as needed
If required, participate in research, program evaluation and data collection for the Family Health Team. All research is to follow the Canadian code of Ethics and privacy law in relation to information gathered. This includes analyzing data and preparing reports as required
Phone patients with results and schedule follow up as instructed by physician
If renewal requested, ensure pharmacy send complete list
Reviews lab results and forwards reports requiring attention to the physician
Other duties that are deemed appropriate and assigned by the Executive Director and FHT physicians from time to time
TEAM RELEATED
Support the mission, vision and goals of the Family Health Team
Participates in health promotion, prevention and chronic disease programming as requested by Executive Director and Lead Physician
Act as a resource to the Executive Director, interdisciplinary team members, physicians and students by researching, providing information and clinical recommendations, advising and conducting follow-up as required
Act as a mentor and supporter to members of the team
Participate in team/committees, task groups and community events
Perform regular chart audits if requested by the Executive Director or Lead Physician
Participates in the orientation process. Provides mentorship, additional skills training for co-workers where requested
Provides supervision of, and/or mentorship of RN/RPN students
Participates in the Performance Development Process with supervisor every year as required and appropriate
Maintains required skills and educational components required specific to area of work
Encourages co-operation between all staff in the work environment
Promotes a safe and healthy work environment
COMMUNITY
Act as a resource and referral link to community groups and team staff by establishing links with other community groups and agencies
Act as a representative for the agency as a whole in all interactions in the community with permission of Executive Director
Promotes team mission statement within the organization and works to achieve goals and objectives as specified in the organization's strategic plan
Promote the team's interest in the community
Participates in ongoing education to ensure ongoing support and positive outcomes
Co-operates with programs working closely with other organizations within the community (Community Care, Public Health, Diabetes Education, Crisis Response etc.)
Has knowledge of, and collaborates with other community organizations and services to improve client care or provide continuing care
Co-operates with other agencies in education regarding services and care available in the community
Organize and/or attend community meetings as a representative of the agency and in support of the Executive Director
ADMINISTRATIVE
Participates in formulation of new policies and procedures, based on evidence based practice and best practice guidelines
Provides input into plans for purchase of all equipment for the Family Health Team
Ensures adequate supplies and equipment are available for daily patient care and emergency situations through collaboration with office manager and/or Executive Director
Assist physicians in seasonal flu and/or injection/vaccination clinic
Ensures he/she is appropriately educated in use of all equipment prior to using
Cares appropriately for all equipment in use in the clinic
Attends meetings and training as required
WOKRING CONDITIONS
Up to 30 hours per week, Monday to Thursday between 9am and 8pm, Fridays 9am-4pm and Saturdays 9:30am-1pm (.75FTE)
