Job Title: Registered Nurse

Reports to the Director of Nursing (DON)/Clinical Manager (CM) and plans, organizes, directs, coordinates and evaluates resident care activities within approved policies and procedures.

RESPONSIBILITIES including, but not limited to;



Assist DON /CM in ensuring that the department is operated efficiently and in compliance with the legislated requirements for Long-Term care facilities in Ontario and other pertinent legislated requirements. Ensure that infection control and occupational health and safety policies and procedures are practiced by nursing staff . Ensures that the care plans are developed, updated as needed, and reviewed as per established schedule. - on PCC using RAI MDS, including coding, RAPS in accordance with MLTC legislated requirements. Supervise and provide direction to RPNs and PSWs, ensuring that resident's care is provided in accordance with the care plans and Home's polices and procedures are adhered to by assigned staff. Review all physician orders and three month med reviews. Ensure Annual Physicals are completed by each physician for assigned residents. Report concerns to Clinical Manager/DON. Attend and actively participate in Resident Care Conferences. Notify Resident/SDM of any changes in health status, falls, injuries, critical incidents, medication change, medication discontinuation, including scheduled care conferences.
Follow-up on resident/families concerns/complaints and prepare reports as may be required to the DON/CM.

Set-up resident clinical charts. Ensure effective admissions; transfers to/returns from hospital, including discharge from the facility in collaboration with the interdisciplinary team;
Work closely with Ministry officials, hospitals, physicians, coroners, community service providers.

Maintain close contact with a hospital on hospitalized residents and ensure the SDM is apprised of changes in resident's health status, including return time to the facility.

Complete MDS as scheduled and provide education to other nursing disciplines as required. Ensure close monitoring and follow-up on ill/high risk residents, including and organized approach in dealing with palliative care residents Ensure that all work is conducted in a safe manner. Investigates/follow-up on all on-the-job injuries, accidents and unusual incident reports, and ensure that required forms are completed according to established procedures, including declared outbreaks/disaster as per PH/Fire Dept instructions. Liaise with attending physicians, psychiatrists, consultants, social workers, clergy, SDM, families, nursing, dietary, activity staff, etc. to ensure effective resident care is provided. Ensure the chart, consents and other required forms are setup for the new admission. Once completed and signed ensure they are filed in designated areas. Ensure referrals are completed when required and other disciplines are notified as needed with respect to changes in resident care needs; Deal with emergency situations. Conduct performance evaluation on RPNs and PSWs as directed by the DON Ensure effective implementation, compliance, monitoring, tracking and evaluation of mandatory programs in collaboration with Nurse Practitioner/Clinical Manager and interdisciplinary care team:

skin and wound care

incontinence care

falls prevention

responsive behaviour

pain management

restraints


Qualifications:
- Valid RN license and registration with College of Nurses in Ontario
- Experience in Long-Term care is preferred.
- Ability to provide compassionate care and support to patients.
- Strong interpersonal skills and the ability to work effectively within a team.
- Excellent communication skills and attention to detail.

Recent (6 months) Vulnerable Sector Police Check or willing to obtain prior to employment.
