St. Joseph's Health Care London is the host organization for the Regional Geriatric Program (RGP) of Southwestern Ontario. Affiliated with the RGP, are Specialized Geriatric Services (SGS) - the clinical programs that provide specialized care to older adults living with frailty in a variety of care settings such as specialized clinics, geriatric day hospital and community outreach. Through interdisciplinary approaches to care, these services support patients and family caregivers by providing comprehensive geriatric assessment, consultation and follow-up. The successful applicant for this position may provide support at any point in the patient's journey with Specialized Geriatric Services including the clinical settings, and also in the patient's home environment. The applicant will primarily support the London-Middlesex sub-region, working closely with Geriatricians, local Geriatric providers in the community, and the Regional Geriatric Program of Southwestern Ontario's Strategy team.
Essential Qualifications
Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Valid Registration with the College of Nurses of Ontario
Valid CPR certificate
Previous relevant inpatient or community-based experience (examples: medical/surgical, rehab, Outreach)
Two years recent experience in geriatric assessment (Including Geriatric Medicine and/or Geriatric Psychiatry)
Excellent problem-solving skills and ability to prioritize in situations of varying degrees of complexity
Ability to function as an independent practitioner and a highly effective team member
Ability to gather medical and social history and clinical data to inform care planning and clinical decision making
Fundamental understanding of physiological and biopsychosocial mechanisms of the aging processes, age-related changes to function, and the impact of frailty
Ability to conduct accurate analysis of assessment findings and clinical information and to integrate assessment findings across geriatric assessment domains, such as falls/mobility, polypharmacy, continence, pain, cognition, and mood
Demonstrated expertise in treatment, education, goal setting, and future planning with older adults and their identified support network
Knowledge of community resources and ability to engage community partners, patients and families in coordinated care planning for community-dwelling frail seniors
Superior interpersonal and relationship building skills
Exceptional computer skills
Exceptional conflict management skills
Demonstrated ability to use best practice and research
Preferred Qualifications
Proficiency in French would be an asset.
Experience with Cerner and Clinical Connect
Knowledge of registration and scheduling
CGN(C) CAN Gerontological Certified
