Position Overview

We are seeking a Registered Nurse (RN) to join our team as a Care Manager . This role is pivotal in ensuring our clients receive comprehensive, individualized care plans and ongoing support. The RN Care Manager will conduct in-home assessments, develop care plans, supervise care aides, and act as a clinical resource for both clients and staff.

Key Responsibilities

Perform initial and ongoing client assessments in-home. Develop and update personalized care plans based on client needs. Provide clinical oversight and guidance to care aides. Monitor client progress and adjust care plans as needed. Communicate with families, physicians, and allied health professionals. Ensure compliance with BC health regulations and company standards. Document all assessments and care plans accurately in the client record system.

Qualifications

Current RN license in good standing with the BCCNM. Minimum 2 years of nursing experience, preferably in community or home care. Strong knowledge of geriatric care and chronic disease management. Excellent communication and organizational skills. Ability to work independently and manage multiple priorities. Valid driver's license and reliable vehicle for home visits.

Preferred Skills

Experience with care planning and case management. Familiarity with assistive technologies and remote monitoring tools. Leadership experience in supervising care teams.

Pay: $45.00-$60.00 per hour
