C-Care Health Services is looking for Registered Nurses (RNs) to be assigned to clinics and home setting in Toronto area.

Scope and Responsibilities

Private and Personal Care + Provide personalized nursing care to the client and attend to their specific needs.
+ Assist client with activities of daily living (ADLs) such as toileting, grooming and mobility.
Nursing Assessments + Conduct initial and ongoing assessments of client's physical, mental, and emotional health.
+ Reassess the client regularly and ensure that the care plan is updated and is still aligned with the client's needs.
+ Monitor client condition and observe any changes in their overall health.
+ Gather and analyze client's information to learn more about their overall health, symptoms, and conditions.
Medication Administration + Administer medications according to the prescribed dosages and schedules.
+ Ensure the accuracy of the medication being administered while monitoring the side effects and allergies.
+ Educate the client about their prescribed medications.
Pre- and Post-Operative Care + Conduct patient assessments before the surgical procedure and ensure good physical and psychological condition.
+ Educate patients on the surgical procedures being performed and give more information on what to expect before, during, and after the procedures.
+ Monitor patients' vital signs and other aspects of physical condition immediately after the surgical procedures including heart rate, blood pressure, oxygen levels, body temperature, etc.
+ Assess and care for surgical wounds through different wound care techniques.
+ Manage patients' pain post-surgery by assessing pain levels, administering medications and other pain management techniques.
+ Provide emotional support and reassurance to the patients by addressing questions and providing a comforting environment.
Documentation and Charting + Maintain accurate and updated patient records.
+ Document assessments, interventions, and outcomes, while ensuring proper communication and continuity of care.
+ Ensure all documents comply with legal and regulatory requirements, maintaining patient confidentiality and privacy.
Safety and Infection Control + Adhere to standard precautions such as proper hand hygiene, use of personal protective equipment as needed, and proper handling of disposal of wastes.
+ Ensure a safe and clean environment through proper disinfection practices and following infection control protocols.
Collaboration and Communication + Collaborate with the Client Care Coordinators and other healthcare professionals to discuss care plans as needed.
+ Collaborate for an efficient transition of care and communicate relevant information to ensure continuity of care.

Minimum Qualifications

Successful completion of a Nursing Diploma program At least 1 year of experience as an RPN Certificate of Competence for Registered Practical Nurse from the CNO BLS or CPR (HCP) Certificate Good verbal and written English communication skills
