Provide direct supervision, leadership, and education to other members of the healthcare team. Acts as a resource and advocate for residents, families, and staff. promote the values, goals, and philosophy of the Home.Responsibilities:xe2x97x8f Development and modification of the plans of care in response to residents' individualized health care needs and change in health care status
xe2x97x8f Liaises with other health care personnel, agencies and families
xe2x97x8f Maintains good relationships with Medical/Support/Home staff, residents, families and general public
xe2x97x8f Supervises all activities on the unit, takes charge as required
xe2x97x8f Monitors performance and completes performance evaluation for the PSW's and RPN's
xe2x97x8f Directs, assigns and evaluates the work of the PSW's
xe2x97x8f May be in charge of Home as required. Utilizes leadership skills to identify problems and problem-solving on the unit
xe2x97x8f Monitors that care is delivered is consistent with the resident Bill of Rights.
xe2x97x8f Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
xe2x97x8f Required to participate in the improvement of the indicators and achieve satisfactory results
xe2x97x8f Orders and ensure nursing supplies and equipment are properly and efficiently used
xe2x97x8f Actively listens to residents and family concerns, take appropriate actions to resolve the issue in accordance with policies of the Homexe2x97x8f Conducts unit rounds at the beginning and end of shift.
xe2x97x8f Processes Physician orders and administers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissions
xe2x97x8f Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the Home
xe2x97x8f Assesses changes in health status and reports changes to Substitute Decision Maker (SDM) or POA, and interdisciplinary team
xe2x97x8f Completes health care assessments (RAI-MDS and other assessments), develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.
xe2x97x8f Demonstrates understanding of the Ministry of Long-term Care mandatory reporting and critical incidents and follow steps outlined in the policies of the Home.
xe2x97x8f Other duties as assigned and or indicated in Job Task InventoryQualifications:The qualifications needed to join our family are as follows:
xe2x97x8f Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
xe2x97x8f Basic CPR
xe2x97x8f Experience in long-term care setting an asset
xe2x97x8f Ability to work with and relate to interdisciplinary team members
xe2x97x8f Well-developed written and oral communication skills
xe2x97x8f Knowledge of RAI-MDS - AIS certified, if not must be completed successfully within one (1) year of being hired.
xe2x97x8f To show evidence of being focused on people's feelings when interacting and knowing how to turn a task into positive social interaction.
xe2x97x8f To undertake and meet all legal requirements within a context of promoting individuals' rights and measuring/reducing elements of risk
xe2x97x8f Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
xe2x97x8f As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements
