Registered Nurse - Community, Medical & Surgical Care
Job Summary:
Modern Angels Healthcare is seeking a dedicated and highly skilled Registered Nurse (RN) with a strong background in medical, surgical, acute, and community health care. The ideal candidate will have hands-on surgical nursing experience, be proficient and comfortable with tracheostomy care in home and community settings, and possess advanced wound care and assessment skills. This role is ideal for a nurse with strong clinical judgment, critical thinking, and a high level of professional accountability.
Key Responsibilities:
Conduct comprehensive patient assessments, including physical, bladder, and genitourinary assessments.
Perform intermittent catheterization and collect urine specimens in accordance with best practices and infection control protocols.
Monitor and report changes in patient condition and vital signs.
Deliver and manage post-operative surgical care, including wound care, drain monitoring, pain management, and recovery support.
Provide complex medical care to patients with chronic or acute conditions in both home and community settings.
Manage tracheostomy care, including suctioning, tube changes, and emergency intervention.
Administer medications and treatments as prescribed, ensuring patient safety and adherence to protocols.
Develop, implement, and regularly update individualized, patient-centered care plans.
Educate patients and families on post-surgical recovery, chronic disease management, continence care, and health promotion.
Collaborate effectively with physicians, allied health professionals, and multidisciplinary care teams to ensure continuity and quality of care.
Maintain accurate, timely documentation
Adhere to all regulatory, ethical, and professional nursing standards.
Qualifications:
Current and valid Registered Nurse (RN) license in good standing with the College of Nurses of Ontario (CNO).
Minimum 5 years of experience in medical-surgical units and acute care settings (hospital-based experience required).
Proven experience in community nursing, including post-hospital discharge care and home-based treatments.
Proficient in tracheostomy care, wound care, intermittent catheterization, and urine specimen collection.
Strong ability in physical health assessments, bladder and genitourinary evaluations.
Demonstrated critical thinking, clinical decision-making, and strong assessment skills.
Proficiency in medication administration, infection control procedures, and electronic documentation.
Familiarity with Cerner or equivalent electronic medical record systems is an asset.
Excellent interpersonal, communication, and organizational abilities.
Ability to work independently and adapt to diverse community care settings with professionalism and autonomy.
