Job Description

We are seeking a compassionate and motivated

Registered Nurse (0.6 FTE)

to join our

Menopause Clinic

in a primary care setting. This unique role offers the opportunity to work alongside a Certified Menopause Educator and a multidisciplinary team to provide comprehensive, patient-centered care. As part of our progressive, or team-based model, you will support women through their menopause journey while working to full nursing scope and applying your clinical expertise in a supportive and dynamicenvironment.


This position offers a choice of schedule:

Monday, Tuesday (12:00-8:15 pm), Wednesday Tuesday (12:00-8:15 pm), Wednesday, Friday


KEY RESPONSIBILITIES



Include, but are not limited to the following:



General

Assessment of patient conditions, needs and goals in collaboration with the patient, attending physician and interdisciplinary team Identify needs and set priorities in collaboration with the client and other team members Provide short term interventions as needed Facilitate appropriate referrals to community resources and programs Provide relevant patient education to prevent and manage clinical conditions Participate in evaluation of the clinic and its services Assist in planning, developing, and monitoring for services provided To assist in the development of policies and protocols for chronic disease management, geriatric care, maternal/child and mental health To develop systems and communications protocols to facilitate patient follow up To assist in setting up health promotion programs relevant to the needs of the practice population. To assist in developing and implementing patient self-management programs. Assist in the implementation of strategies for improving the quality and provision of care for the practice population, Any other duties that may be reasonably required for the success of the PCN.


EDUCATION & EXPERIENCE



Baccalaureate degree in Nursing preferredFive years or more of related nursing experience, Current CRNA Registration, CPR, and BCLS, Initiative, accountability, decision-making, interpersonal, and communication skills, and Demonstrated ability and willingness to work in a dynamic environment that requires flexibility and balancing of competing priorities. Experience working in a team setting with other health professionals, Advanced skills in caring for patients with chronic, complex, and/or specialized needs Experience in establishing protocols for program startups, especially developing plans and priorities when workload varies Expert knowledge of nursing processes and practices including intervention strategies and therapy Strong, effective written and oral communication skills, high level of perceptiveness, Critical thinking skills Ability to build a good working relationship with healthcare professionals, clients and other members of the community Ability to prioritize, manage time effectively and be flexible in a very active work environment. Respect and understanding of confidentiality, A strong knowledge base in medications and is able to conduct medication reviews with patients Customer service orientation, Thrives in a complex changing environment, High levels of integrity and professional behaviour At least basic computer skills in word processing and email. Knowledge of, and ability to use, electronic medical records

OTHER OCCUPATIONAL REQUIREMENTS

A satisfactory Criminal Records Check with Vulnerable Sector. This would be the financial responsibility of the successful candidate. A valid drivers licence and access to a vehicle for travel to various stakeholder locations Will require flexibility in hours and days of work with frequent early mornings and evening hours. Weekend time is unlikely at this time, but may change depending on stakeholder availability
