The qualified Registered Nurse will be a member of the interprofessional Geriatric Care Team providing assessment, care, and treatment in the community setting. This is a permanent position and will report to the Executive Director.
Duties Include:
The RN will be a member of the interprofessional Geriatric Care Team providing assessment, care, and treatment in the community setting. In collaboration with other team members, the RN will:
Assess, plan, implement and evaluate care of elderly persons with complex geriatric health problems
Provide consultation to health care professionals and elderly patients regarding geriatric health issues
Collaborate with health care professionals to develop and implement a comprehensive plan of care
Assess the coping strengths of the elderly person and his/her family/support and plan intervention and/or intervene to assist the elderly person to achieve optimal independent functioning
Provide health education and health promotion information to patients and their families
Involve patients in decisions about managing their chronic disease(s)
Link with community resources and share information to provide the best experience for the patient and their family
Be comfortable delivering education and care virtually and in-person
Provide home visits when needed
Ensure all documentation is completed in a timely manner
Maintain and upgrade knowledge and skills related to geriatric RN role
Qualifications:
Current College of Nurses of Ontario Certificate of Competence in good standing
Current B.C.L.S.
Working knowledge of community resources, or a willingness to learn
Experience in specialized geriatric programs and CNA Specialty Certification in Gerontology are assets
Demonstrated ability to function on an interdisciplinary team
Knowledge in functional assessment and the assessment of physical and mental illness
Excellent organizational skills, problem solving and time management skills
Exceptional documentation skills and attention to detail
Ability to adapt to change, innovate, and be flexible in an ever-changing environment
Valid Ontario driver's licence with access to reliable vehicle
Successful completion of a vulnerable sector check
