Seeking a dedicated In-Charge Registered Nurse (RN) to lead clinical operations in a 90-150 bed long-term care home. The RN will oversee Peritoneal Dialysis, InterRAI LTCF assessments, resident care, staff supervision, , and medication administration using PointClickCare. This role includes managing peritoneal dialysis, infection control, emergency response, and staff training.
LICENSE/CREDENTIALS
Completed Inter-RAI LTCF certified (required)
Current Registered Nurse Certificate in good standing with the College of Nurses of Ontario
Basic CPR, Stable Work History, Highly Competent in medication administration
Minimum 2-year experience as In-Charge Nurse of Home in long term care setting an asset
Demonstrated ability to lead, manage and take charge of 90 - 150 Resident Bed Home
RESPONSIBILITIES

Inter-RAI LTCF & PCC
Responsible for Inter-RAI LTCF assessment, coding and compliance assessment
Exceptional documentation & charting skills required with a strong understanding of PointClickCare & InterRAI LTCF
DIALYSIS:
Comprehensive Knowledge of Peritoneal Dialysis
Responsible for leading, manage and provide Peritoneal dialysis on all floors
Must troubleshoot peritoneal dialysis issues and collaborate with SMH Homedialysis team.
OUTBREAK/EXCEPTIONAL CIRCUMSTANCES:
Required to be work on-site, if an outbreak (eg respiratory/COVID-19) occurs at the Home
Responsible for IPAC Audits
Responsible for assisting IPAC Lead in maintain Infection Control practices
DOCUMENTAION & OTHER DUTIES
Oversee G-Tube/IV process
Required to do respiratory/cardio/wound assessment/physical assessments of all/any residents
Completion of audits assigned for all floors/POC audits
Required to provide Nursing related in-services/ training to all staff as assigned by DNS/DOC
Dining supervision and assistance for all floors

Responsible for Enacting all emergency codes and plan
Responsible for Staff schedule when necessary
Excellent computer and basic software skills
IN-CHARGE/ LEADERSHIP
Must provide leadership support to the Director of Nursing Services/Director of Resident Care.
Effectively communicate, supervise and manage conflict amongst staff, residents and visitors to home including contractors and family members
Able to work in collaboration with other LTC departments (Dietary, Reception, Physio, Environment etc.)
Must lead in extenuating circumstances & react to emergency situations appropriately as and when required
Able to confidently liaise with family members regarding resident care & able to multi-task
Responsible for doing safety rounds in the home and reporting any issues to ESM/E.D
Responsible for reporting unusual occurrence events as per MOH guidelines to DNS/ E.D.
Will be required to provide Nursing coverage for Resident Care Units if required

POSITION SUMMARY:

Should the DNS and DRC be off-site, this position holder will be responsible for the Nursing Department of the Home, under guidance from the Director of Nursing Services.
As RN Charge Nurse, the staff will co-ordinate and oversee all aspects of resident-focused delivery of care. Act as a resource and advocate for residents, families and staff. Priority one will be quality care, customer service and employee relations. In addition, the supervision and career coaching of existing team members on staff. Timely completion of InterRAI LTCF assessments. Development and modification of the care plans in response to residents' change in status. Administers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissions. Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of Home. Participates in the admission, discharge, transfer of the resident to/from unit. Full Position responsibilities to be followed as per The Rekai Centres Human Resources Manual.
REPORTING RELATIONSHIP:
Reports directly to the Director of Nursing Services and Director of Resident Care.

Physical fitness: Repetitive bending movement at/above/below waist/shoulders/ knees for extended periods of time. Pushing/ pulling movement, periodically lifting may be required, swift walking etc.
Ability to maintain a calm and composed demeanor in a high attention demanding environment
Recognized Leadership amongst PSWs, RPNs and RNs
