Job Description

We are looking for a compassionate and skilled Registered Nurse (RN) to join our primary care team. The RN will deliver nursing care in accordance with Richmond Medical Clinic FHO policies and professional practice standards, and according to regulations set out by the College of Nurses of Ontario. The RN will work alongside the primary care team to deliver comprehensive nursing care, support chronic disease management, and address the social determinants of health.

Responsibilities



Assess, plan, implement and evaluate nursing care patients, in consultation with the physician, of the Richmond Medical Clinic Interpret patient health records, results and testing Maintain complete and accurate medical records Assist the primary care team (assessment, treatment and follow up) and perform clinical tasks by conducting nursing assessments, health education/counselling, history taking, performing specific procedures according to the CNO standards of nursing practice and providing follow up care. Maintain competence of treatment of illness and nursing practices Facilitate patient/family education to promote health and wellness Participate and provide support in chronic disease, mental health, well babies, Administer medications, immunizations, vaccines and treatments as prescribed by a physician and according to established policies, procedures and/or medical directives Participate in the development and implementation of Richmond Medical Clinic programming Participate in problem solving to enhance patient care Develop and deliver health education programs for patients, specifically to diabetic patients and in well baby appointments Initiate and help patients identify and use health resources, including those available in the community and from community agencies Other duties as required/as assigned

Qualifications



Demonstrated competence in nursing practice as a Registered Nurse Registered Nurse (BScN) and current member in good standing of the Ontario College of Nurses of Ontario Current CPR Certification Excellent team work and interpersonal skills Demonstrated ability to prioritize multiple competing demands and to manage challenging situations effectively Demonstrated excellent critical thinking, communication (verbal & written) and interpersonal skills required Demonstrated flexibility, creativity, and ability to prioritize Ability to accept direction and feedback Preference will be given to those with experience in diabetic education
Job Types: Full-time, Permanent

Pay: $33.00-$37.00 per hour
