Your Opportunity:
Under the direction of the Patient Care Manager the Registered Nurse - AKC-N Patient Navigator is to implement, revise, and maintain a program designed to focus on navigating patients with chronic renal failure to renal replacement treatment options. In collaboration with other members of the health care team, the Registered Nurse (RN) supports the organization of appointments and/or coordination of services including but not limited to dialysis services closer to home community, referrals for home dialysis, and referrals to other providers, services, and resources. The RN works to promote care continuity and the reduction of system fragmentation in the patient and family's chronic kidney disease journey. The RN triages new and returning patients and establishes a therapeutic relationship, providing a single point of contact to link the patient, health team, hospital, and community services, to support care coordination and continuity through key transition points along the kidney care continuum. The RN will work collaboratively to promote optimal dialysis starts and home modality treatment options. The RN is responsible for identifying and reviewing the clinical data necessary to support QI and research projects; Monitoring, inputting and reporting on QI outcomes and measures; ensuring adherence to QI indicators; monitoring of home dialysis patients and increasing the number of patients choosing home dialysis. The RN will employ comprehensive chart reviews to ensure safe patient transitions and support capacity planning of inpatients where necessary. The RN collaborates with patients to coordinate their dialysis needs. The RN will draw from a strong background in home dialysis and hemodialysis therapy to provide patient teaching. This position is based in the Patient Navigation office at the University of Alberta Hospital.

You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.
