Reporting to the manager of the Forensic Assessment Outpatient Service, the Registered Nurse - FAOS CTO Coordinator/ Case Manager will be part of a multidisciplinary team functioning with the primary responsibility for the triage, coordination and clinical case management of individuals placed on Mental Health Community Treatment orders. You will provide both community clinical outreach and in office clinical case management of same for FAOS. You will attend and participate in post referral assessment and suitability for Forensic services delivery. You will be responsible for the outreach and in hospital case consultation and management prior to program acceptance. Medication and clinical management, coordination of client apprehensions, appeals and renewals. The incumbent will carry a clinical case load and coordinate care in conjunction with the attending psychiatrists. Utilizing the strengths of the multidisciplinary team you will work with the clients to achieve and maintain optimal levels of health and wellness. This position has transitioned to Recovery Alberta effective September 1, 2024. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
