You will be:

Available to work on days and evenings Monday to Sunday.

Available to work every 2nd weekend.

What You'll Be Doing:

Deliver comprehensive nursing care to patients in home settings, focusing on individualized treatment plans.

Conduct thorough assessments of patients' health status and develop appropriate nursing interventions.

Collaborate with multidisciplinary teams to coordinate patient care and ensure continuity of services.

Educate patients and their families on health management, medication administration, and home care practices.

Monitor patient progress and adjust care plans as necessary based on clinical evaluations.

Maintain accurate documentation of patient care activities using electronic health record systems, such as Epic.

Provide support for laboratory tests and procedures as needed, ensuring adherence to safety protocols.

What we look for in our candidates:

Previous experience with pediatrics is highly desirable.

Strong background in patient care with a focus on community home health (3 years experience in the community) will be a strong asset.

Excellent communication skills and the ability to work collaboratively with healthcare teams and families.

Experience working with children and understanding their unique healthcare needs is beneficial.

Experience with CPAP, BiPAP, and home ventilators.
