We currently operate two well-established clinics in Scarborough and Richmond Hill. Beginning in Fall 2025, we will be expanding with five new locations to support continued growth and community needs.

Registered Nurse / Registered Practical Nurse
to join our Family Health Team.
Terms:
Full Time Permanent / Part Time Permanent / Casual
Responsibilities:
Implement and coordinate a management system and interventions in support of the Chronic Disease Management and Prevention programs through outreach, health education, individual counselling, and virtual care
Provide clinical support while working closely with others in a multidisciplinary team
Engage in and facilitate activities under health promotion/teaching/prevention such as group education sessions and community outreach
Liaise with care providers in the health care system and facilitate referrals to other services and resources within the Family Health Team, secondary and/or tertiary health care centres
Ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Nurses of Ontario and organizational policy, practice and values.
Demonstrating commitment to professional life long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession.
Performing reflective practice, analyzing and evaluating nursing knowledge and modifying practice accordingly to improve the quality of patient care.
Performing cross-functional and other responsibilities, as assigned and/or requested
Promote positive public relations and working relations in a family-centered care environment
Provide administrative and clinic management support including but not limited to providing clinical coaching/support to the team and ordering and maintaining clinical/lab supplies, medications, vaccines, crash cart, PPE inventory, etc.
Maintain and manage patient files, charts, and records while prioritizing patient confidentiality
Requirements:
Bachelor of Science in Nursing or RPN diploma from an accredited institution
Registered and in good standing with the College of Nurses of Ontario
Valid CPR HCP certification
Minimum 2 years of clinical experience in a community or primary healthcare setting
Demonstrated experience in the area of chronic disease management/ health teaching
Exceptional telephone etiquette and customer service skills
A team player with a high degree of accuracy and attention to detail
Strong interpersonal skills, with the ability to accept work from various sources (physicians, nurses, team leads)
Well organized and capable to multitask in a fast-paced environment
Proficient computer skills in Microsoft Office, and EMR (Telus Practice Solutions) is an asset
Available to work flexible hours including some evenings and Saturdays
Available to work at both the Scarborough and Richmond Hill clinic locations
Fluent in English and a second language that reflects the community served, such as Cantonese and Mandarin, is an asset
A valid vulnerable sector police record check completed within one year
