DESCRIPTION:



The Registered Nurse (RN) is a key member of the multidisciplinary team who provides services in a manner consistent with KHSC Mission, Vision and Values and that reflects the core values of Patient- and Family-Centred Care (dignity, respect, information sharing, participation, and collaboration); support a healthy workplace that embraces diversity, encourages teamwork, and practices in accordance with the standards of professional practice and ethical values as defined by the College of Nurses of Ontario.


The RN works as a member of a multidisciplinary team in the Adult Mental Health Program, which offers non-urgent and urgent short-term intervention outpatient services including assessment, consultation, pharmacological treatment and group psychotherapy for adults aged 18 years and older with one or more mood and anxiety disorders; uses advanced skills in assessment, consultation, treatment and psycho-education to deliver innovative client-centred services to individuals and families; utilizes an effective scope of clinical practice for direct service, consultation, program development, teaching and evaluation; includes collaborative interdisciplinary strategies in their professional practice that promotes effective problem solving; and participates in the planning, presenting and evaluating of psycho-educational information and programs; collaborates and communicates effectively with patients, families and all members of the health care team of multidisciplinary professionals and support personnel; participates in service and hospital wide activities and programs that model and promote a safe and healthy patient-centred environment and that contribute to quality improvement and risk management initiatives. The RN will support AccessMHA by completing thorough standardized mental health and substance use health assessments with more complex individuals in a more in-depth standardized assessment appointment. The RN will work within a stepped care framework to match to appropriate mental health or substance use health programs within the suite of service offerings with AccessMHA partners. This role supports connection to Peer Support within AccessMHA, connects the client to waitlist supports and/or provides service navigation to external service when necessary. The role of the RN is also to support and provide guidance to assist the work of AccessMHA Information Specialists. The RN is responsible for developing a service summary report and any appropriate documentation that is provided to the Primary Care Provider.



The RN is responsible for ensuring that practice and conduct meets legislative requirements and the standards of the profession as designed by the College of Nurses of Ontario. RNs' practice will also comply with Kingston Health Sciences Centre's policies and procedures. Within this role the Registered Nurse is accountable for contributing to the delivery of the KHSC strategy. This position is primarily assigned to the Adult Mental Health Program with service provided to HDH-Mental Health and Addiction Services


PRIMARY RESPONSIBILITIES & DUTIES:


The RN initiates and coordinates inter-professional team assessments and plans of care including physical, spiritual, and psychosocial needs of patients across the continuum of care. Delivers care and provides care coordination for an assigned group of patients in an ambulatory and/or inpatient setting. Plans, organizes, and establishes priorities based on individual patient's needs and scheduled activities and adjusts priorities and activities in response to unanticipated events. Develops plan of care to achieve identified patient goals within the ambulatory and/or inpatient setting and including planning for transitions within and from the hospital setting. Provides teaching and learning experiences for both patient and family. Coordinates family communication, family meetings and conferences. Provide clinical nursing skills (triage and assessment) to the Regional Coordinated Access service (AccessMHA) for Ontario Health-East. Conducts mental health and substance use health screening, triage, and assessment for those referred to AccessMHA, including professional and self-referrals, utilizing standardized tools protocols and processes Using a stepped care framework, works with the client and/or referral source to match to an appropriate service within the suite of AccessMHA service provider partners If service needs cannot be met within service offerings, assists with service navigation to an appropriate external service, assisting with referral process for external service when necessary/appropriate Supports connection to wait list supports (such as online supports, self-help groups, peer support) as required Connects client to peer support within AccessMHA as appropriate Connect family members to supports as appropriate Performs therapeutic measures as prescribed by members of the professional care team and as indicated by the nursing diagnosis. Maintains appropriate records of observation, nursing care administered and patient's response. Communicates regularly with the interprofessional team members throughout the patient care experience. Assigns, teaches, and supervises Patient Care Assistants/Volunteers. Acts as the key contact person for new interprofessional team members and assigned group of patients. Provides for an environment which is conducive to the safety and well-being of patients, visitors, and staff by practicing and promoting accident prevention; by being familiar with emergency procedures and regulations; by using skills, physical facilities, supplies and equipment safely and appropriately; and by being alert to infection hazards and infection control procedures. Maintains competence in nursing practice through in-service and continuing education. Participates in nursing research. Serves on departmental and related committees when and as requested. Interprets nursing department and hospital policies to patients, families and visitors. Performs related work as requested.

BASIC QUALIFICATIONS:


Registered in good standing or eligible for registration (within 3 months of start date) with the College of Nurses of Ontario 2 years' experience in mental health nursing One year of recent related experience in Mental Health within the last five years required Excellent skills in assessment, risk assessment, and crisis intervention techniques Experience working in an interdisciplinary service delivery model is required Collaborative approach to working with varied stakeholders and partners, and excellent advocacy skills Demonstrated ability to use effective decision-making within a complex clinical environment Ability to work under pressure and meet tight deadlines and benchmarks Has working knowledge of provincial and national telemedicine standards, policies and guidelines and is comfortable and skilled working in a virtual context and ability to work in an automated work environment, Proficiency in speaking and comprehending both English and French Demonstrate knowledge, skill, and ability in dealing effectively with patients with severe mental illness. Strong knowledge of, and ability to apply principles of crisis intervention; and an understanding of psychiatric nursing as appropriate for this patient population is required. Highly developed clinical skills in interviewing, psycho-social and risk assessment, treatment, and crisis intervention with patients and families Demonstrated knowledge of DSM-V TR diagnostic criteria and psychopharmacological - somatic therapies, community based and regional mental health and other relevant resources Demonstrated ability to effectively conduct a needs assessment, develop collaborative care plans, educational tools and programs and facilitate individuals learning experiences for patients Excellent time management, organizational and prioritization skills Proficiency with Microsoft Office applications including Outlook, Word, Excel Capable of coping in a physically and mentally demanding work environment Ability to function independently as well as interdependently Proven ability to make critical decisions independently and rapidly & strong conflict resolution abilities Demonstrated knowledge of Ontario Mental Health Act, Health Care Consent Act, Powers of Attorney Act, Substitute Decisions Act and other relevant legislation Demonstrated interpersonal and communication skills (both oral and written), strong conflict resolution abilities, and ability to establish and maintain good working relationships coupled with the ability to work on a multidisciplinary team Knowledge and competence in CPR.
