Responsibilities:
The successful candidate will work within the Home Dialysis Program training home patients to perform their own dialysis
Candidates will be required to visit to the patients in their homes as part of the training and support
Candidates will be expected to assume on-call duties
Qualifications:
Graduate of a recognized College or School of Nursing
Baccalaureate degree preferred
Current certification of competence from the College of Nurses of Ontario
C-Neph (C) designation preferred
Current BCLS
Minimum 2 years experience in Hemodialysis or Peritoneal Dialysis preferred with the completion of the vascular access modules from the Ontario Renal Network
C.A.P.D. and APD (Cycler) experience an asset
Excellent leadership, communication and critical thinking skills assessment, critical thinking and
Demonstrated ability to apply the principles of adult learning while teaching patients and families
Demonstrated ability to be self-directed and work independently
Commitment to ongoing professional development within an evolving field
Ability to apply the principles of adult education and demonstrated teaching skills required
Candidates must provide their own transportation for visits to patients homes and between THP sites
Availability to work all shifts and flexibility in work schedule is required to meet patient's needs for training and home visits
