Working independently, and participating as a member of a team-based care, the Registered Nurse in Primary Care is responsible for direct nursing patient care at Lexitor Medical. The RN partners with and complements members of the primary care team. The RN focuses on providing assessment, screening, healthy lifestyle support, education, and chronic disease management with a goal of improving health outcomes and facilitating access to services for the entire patient population. The RN in primary care practices in accordance with the professional and practice standards and code of ethics as outlined by the BC College of Nurses and Midwives and applies best practices in accordance with established organizational policies, policies, procedures, and guidelines to provide competent, safe, and ethical care. The RN provides person and family-centered care with cultural humility using evidence-based and trauma-informed best practices. The RN must be comfortable working with maternity patients and babies, as well as people of all ages.
TYPICAL DUTIES AND RESPONSIBILITIES
1. Within the RN scope and role, as part of a collaborative team (physicians, social worker, mental health worker and pharmacist), provides quality care for people at al stages of life including:
- Addressing chronic disease management, preventative screening, general health education, sexual health, wound and skin care, advanced care planning, emergency assessment and care, point of care testing, specimen acquisition and arranging referral where necessary.
- Assessing, diagnosing, planning, implementing, documenting, and evaluating the care needs and goals for individual patients, caregivers/families, and communities.
- Planning and participating in patient screening and recall strategies.
- Providing procedures such as ear flushes, immunizations, liquid nitrogen wart treatments.
- Telephoning patients to explain results.
- Immunizing children and adults
- Acting as patient/community advocate.
- Promoting and facilitating patient independence
2. Conducts primarily in-person patient care, with a small amount of virtual and home visits as applicable, to monitor the care being provided in a safe, efficient, and effective manner, ensuring the achievement of identified service goals
3. Acts as a mentor to resident physicians, medical students, nurses, or nurse practitioners training in the practice. Model Lexitor Clinic values in interaction with others such as demonstrating respect, trust, and cooperation and recognizing personal impact on team dynamics.
4. Participates in the maintenance of an accurate comprehensive health record. Documenting nursing care and nursing interventions according to BC College of Nurses and Midwives (BCCNM) documentation practice standards; including intake screens, client assessments, treatment plans, progress notes. Protects and secures all patient records and supplementary related documents by monitoring access and maintaining appropriate disposition of the documents/information to protect confidentiality. Provides patient data to specified healthcare professionals.
5. Processes and implements the care plans and orders of listed health professionals, in accordance with regulatory requirements, identifying and seeking clarification as necessary. Coordinating care provision with other programs/services.
6. Participates in case conferences, treatment plan reviews, emergency drills practice, and staff meetings.
7. Contributes to a safe and healthy working environment by observing and promoting universal precautions and infection control procedures; removing obvious hazards; reporting faulty equipment, accidents, injuries and near misses; and participating in quality improvement.
8. Ensures competency and professional practice standards are met, in line with the regulatory controls on practice.
9. Advocates for education, continuing professional development, and training as required to maintain and expand competence to meet patient needs.
10. Performs other related duties, as assigned, within scope and role.
QUALIFICATIONS
Education, Training, and Experience
- Registration with BC College of Nurses and Midwives as a practicing RN registrant in good standing.
- A minimum of 5 years of recent related primary care, maternity or community/public health experience, knowledge of available programs and services, and their function in delivering care in the community; or an equivalent combination of education, training, and experience.
-Valid BC driver's license
