We are seeking a dedicated and compassionate Community based Registered Nurse to join our healthcare team. The ideal candidate will have a strong background in patient care across various settings, including hospital, nursing home, assisted living, and home care environments. This role offers an opportunity to provide high-quality nursing services, utilizing advanced skills in medical procedures and patient management. The Registered Nurse will play a vital role in delivering comprehensive care to diverse patient populations, including pediatrics, seniors, and individuals with dementia or chronic conditions. Experience with electronic health records such as Epic and Cerner is preferred to ensure seamless documentation and communication within our healthcare system.
Duties
Provide direct patient care in hospital, nursing home, assisted living, or home care settings
Administer medications accurately and safely, following prescribed protocols
Perform phlebotomy procedures and laboratory specimen collection as needed
Monitor patient vital signs and assess health status regularly
Develop and implement individualized care plans in collaboration with healthcare teams
Manage pain effectively through medication administration and non-pharmacological methods
Assist with case management activities to coordinate ongoing patient care
Document all patient interactions accurately using electronic health record systems such as Epic and Cerner
Educate patients and families on health conditions, medication use, and post-discharge care instructions
Recognize and respond promptly to medical emergencies or changes in patient condition
Maintain compliance with healthcare regulations and safety standards
Requirements
Valid Registered Nurse (RN) license in the state of practice
Experience with hospital acute care, senior care, dementia care, pediatrics, or home health preferred
Proficiency with electronic health record systems such as Epic and Cerner is highly desirable
Strong knowledge of medical terminology, medication administration, and nursing procedures
Experience with pain management, case management, laboratory procedures, and medical office operations is a plus
Excellent patient care skills with a compassionate approach to diverse populations including children and seniors
Ability to work effectively in fast-paced environments requiring critical thinking and quick decision-making
Prior experience in nursing homes or assisted living facilities is advantageous
Knowledge of medical procedures related to phlebotomy, medication administration, and laboratory testing
