Cota is an accredited not-for-profit organization that has been providing community-based support to people living with mental health and cognitive challenges for over 50 years. At present, our wide array of services delivers much needed support to over 4,000 people/year who live with mental health challenges, psychogeriatric mental health conditions, acquired brain injuries, developmental disabilities, dual diagnoses and/or the experience of homelessness. Role Overview
The Registered Nurse, Mental Health Case Manager provides case management support and care for the over all well-being of clients. The Registered Nurse, Case Manager will provide support to clients in their homes and community settings in North York Region.
About the Team
ARCS is a hospital diversion program in partnership with North York General Hospital and Humber River Health. The objective of this program is to reduce emergency department visits, by providing short-term, community based case management services to individuals that had recently visited the emergency department for mental health and/or addiction matters.
The ARCS team is interdisciplinary, consisting of Mental Health Case Managers, Addictions Specialist Case Managers, a Peer Support Specialist, and an Intake Case Manager.
Primary Responsibility:
Responsibilities of this position include but are not limited to those below:
Clinical Skills:
Reviewing the referral information of applicants to the program
Determine eligibility for the program and confirm acceptance of new clients into the program
Developing a collaborative, supportive and therapeutic relationship
Completing a comprehensive psychiatric and physical health assessment of the nature of each client's unique goals and needs through collaborative engagement with each client and their respective support networks
Providing guidance and assistance to each client in relation to the development of both a personalized Client Safety Plan and a Recovery/Goal Plan
Provides health education to clients and team to promote health and well being
Provide direct practical support, coaching and counsel to clients in their pursuit of identified goals and needs.
Assisting clients with emergency and/or crisis needs as required/appropriate
Documentation and Data entry:
Documents all client contacts/interactions in the appropriate client chart according to Cota's documentation guidelines and College standards
Assists with other data collection activities within the program, as determined by the Program Manager
Administrative tasks:
Participates in team meetings, provides input and ideas for improving services, efficiencies and ongoing program planning
Participates in any research-related activities
Qualifications:
Education:
A Bachelor's degree in nursing from a recognized University and must be registered with College of Nurse of Ontario
Experience:
The ideal candidate will have 2 years direct experience in community mental health nursing however new grads are welcome to apply with case management and mental health experience.
Sound clinical knowledge and experience in case management with the client population, including individuals experiencing serious mental health issues and homelessness.
Experience working in a community agency an asset
Knowledge & Skills:
Demonstrated strong commitment to the right and ability of each person with a serious mental illness to live in normal community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services
Conduct home visits and to exercise judgement with a high degree of autonomy
Establish supportive, trusting relationships with persons with a serious mental illness with respect for clients' rights and preferences
Use problem-solving techniques to identify and assess client need and develop appropriate intervention strategies
Assess mental health needs, physical health needs, functional capabilities and care options
Ability to make appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric and medical treatment
Offer groups and individual services to educate clients, families and other staff about health and wellness behaviours (e.g., diabetes management)
Handle ambiguity to function effectively while dealing with competing priorities
Familiarity with the Ontario Mental Health Act and Mental Health Reform
Knowledge of Recovery as a guiding principle for assessing and developing treatment plans for clients
Ability to work with agencies, hospitals and the broader community
Knowledge of trends and issues related to the delivery of services to a mental health client population
Excellent organizational and interpersonal skills
Excellent computer skills (e.g., Word, Excel, Microsoft Outlook)
Ability to manage and adapt positively to an ever changing health care/work environment
Effective communication and written skills
Other:
Visits will be conducted in the clients' homes, hostels, hospitals and other community settings
Weekend and evening rotation may be required
Must have access to vehicle (Please note that access to a vehicle means owning, borrowing, or renting the vehicle. Alternatively, using ride services, such as Uber, cab, and taxis at no cost to Cota)
