The Diabetes Nurse Educator is responsible for delivering and maintaining a Chronic Disease Education Program that is focused on improving the health and quality of life of individuals with Diabetes/Hypertension/Dyslipidemia and Cardiovascular disease (CVD) risk factors. The overall purpose is to offer excellence in education and direct clinical care through a multidisciplinary environment. The Nurse Educator assists the individual and his/her family by facilitating learning and self-care through various roles. Direct client care is provided to adults and children who are diagnosed with type 1 diabetes, type 2 diabetes, and gestational diabetes and may be managing their chronic disease with various elements of diet/lifestyle, medication management, intensive insulin management including insulin via pumps and the use of sensor technology. This position will be part of Primary Care Alberta effective November 18, 2024. As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.
