The Markham Family Health Team (MFHT) serves over 27000 patients and has three sites in the Markham area. Our team consists of 19 Physicians and a wide variety of Interdisciplinary Health Professionals and Administrative Professionals. (See website for more MFHT information).
We are currently looking for a full time Registered Nurse to join our team. Part of this role involves assisting in the delivery of care through case coordination and system navigation, as well as working in various Health Team programs. This role may also include some visits to the patient's home.

Position Overview
The role of the Registered Nurse, within the Markham Family Health Team (MFHT), requires a thorough knowledge of primary care nursing and the ability to function effectively in a multi-disciplinary team environment. The role also requires a high level of interpersonal skills to advocate for patients and their families in a compassionate manner. The position involves leading the delivery of health promotion and chronic disease management programs and requires strong critical thinking skills. Early identification of care needs plays an important role in promoting, planning, and achieving an optimal level of function at home and in the community. Experience or interest in working with older adults is essential. This supports the MFHT vision, values, and mission.
The RN will play an integral role within various MFHT programs, utilizing assessment skills to gather, analyze, and develop a plan of care to best meet the needs of patients and their families. The RN will assist in the coordination and delivery of services by liaising with various stakeholders both internal and external to the organization.
The RN role will also support the MFHT INR anticoagulation program. This program includes in office point of care testing and use of Medical Directives to guide decision making for this unique cohort of patients. Experience working with Directives is essential.
Roles and Responsibilities
Chronic Disease Management:
Assess patient's health status, provide and evaluate care in-office
Summarize the focus for the patient's care following consultation
Work collaboratively with the other team members to optimize patient care
Deliver information to patients and families through active participation in family conferences
Systematic follow up phone calls and/or visits to support and reassess on-going patient needs
Ensure all patient resources are current, evidence-based, and up to date reflecting cultural competency and diversity and inclusion
Provide care to patients utilizing Medical Directives including dosage adjustments for patients on warfarin.
Education and Program Involvement:
Educate patients and families on how to use/find health resources, and how to navigate the health system
Participate in team building, program development, health promotion and improvement initiatives
Participate in staff meetings to exchange ideas, support and monitor program outcomes
Attend relevant conferences, workshops, or other in-service events
Active role in the MFHT through attendance of rounds, workshops, etc.
Acute care:
Triage of patients based on acuity of presenting problems, concerns and chief complaints
Procedures including immunizations, injections, and dressings
Organizational Support:
- Assist and participate in various treatment initiatives and activities
-Comply with established personnel policies, procedures, standards, and guidelines for FHT operations
- Apply full RN scope of practice through contribution to other FHT initiatives as necessary
Qualifications and Requirements
RN Diploma or BScN
Entitled to practice with no restrictions as per the College of Nurses of Ontario current membership
Current membership with the RNAO.
Minimum two years direct patient care experience, preferably with geriatric and medicine experience
CPR certification
Ongoing education to support role of RN
Valid driver's license and own vehicle
Skills and Knowledge:
Involve patients in decisions about their own health and teach how to self-manage care
Initiate and support health education activities
Ability to explain/translate medical language
Experience utilizing Medical Directives to guide practice
Excellent interpersonal skills
Courteous and professional telephone manner
Strong time management, detail-oriented and organizational skills
Proficient computer skills
Ability to develop trusting and respectful relationships
Proven ability to work in cross-cultural environments
