Geriatric Outreach Program

Parkwood Institute Mental Health Care - London, ON

Temporary Full Time

Salary Range: $39.07 - $56.00 /hour

Temporary position anticipated to extend until March 31, 2026, and is subject to the availability of work.



The primary role of the RN is to provide high quality, safe, effective and evidence-based care in partnership with patients, families and community partners in accordance with the College of Nurses of Ontario standards.




The RN on the Geriatric Outreach Team operates within an outpatient environment supporting both clinic-based appointments as well as, community visits with patients and their caregivers. Using the principles of client-centered and recovery-oriented care, the successful candidate will provide supportive treatment for older adults with severe and persistent mental illness.





Essential Qualifications



Valid registration with the College of Nurses of Ontario Valid CPR certificate Well-developed assessment, consultative practice and evaluation skills in the care of older adult's clients with severe and persistent mental illness Ability to carry out comprehensive geriatric assessments utilizing standardized tools Strong knowledge of complex psycho geriatrics issues and multiply medical comorbidities Knowledge of Pharmacological and non-pharmacological interventions commonly used (ex ECT, TMS, PST, psychotherapy, and supportive therapies) Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA Knowledge of neuropsychiatric conditions and treatments Ability to identify those at risk of mental health issues, develop a risk assessment and appropriate care plan Must be capable of establishing positive interpersonal relationships and able to work as an effective team member Demonstrated ability to apply and document the nursing process Ability to relate therapeutically with clients Demonstrated leadership in the initiation, coordination of, and participation in team/family conferences and clinical activities Ability to be autonomous and manage and triage priorities Knowledge of community resources and partners (BRT,BSO,DLT,CMHA, SGS, etc Excellent communication and interpersonal skills Excellent organizational, critical thinking and problem solving skills Experience in the development of teaching materials and the teaching of clients, family, and caregivers Ability to apply effective body mechanics and meet the physical demands of the position Ability to travel within the catchment area

Preferred Qualifications

Proficiency in French would be an asset. Experience working with the elderly and/or in mental health BScN Certified in, or in process of completing courses toward the certificate in Gerontology and/or the Canadian Psychiatric and Mental Health Nursing Certification GPA, Piece, Asist and PST trained

Immunization Requirements

Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza. Provide documentation of the Tuberculosis skin testing
