Job Description

Job Summary


The RN Navigator is responsible for working in collaboration with physicians to provide symptomatic, psychological/social, and navigational support to patients and families throughout the palliative care trajectory. The RN will work within the Palliative Remote Care Monitoring program, leveraging technology to support patient-centred palliative care in the community.

Reports To: Executive Director

Position: 2 days per week (0.4 FTE) from December 2025 - March 2026,

with possibility of extension dependent on funding

Responsibilities:



a) Provide support and liaise with to physicians and other practitioners to facilitate patients' seamless and timely progression through the health care system; follow up regarding appointment completion/attendance and subsequent results.

b) Provide ongoing support and follow up to patients before, during, and following treatment; for navigation of treatment and to maximize health potential.

c) Provide palliative care support to help prepare for end of life by supporting patients and their families, ensuring questions are answered, and developing a plan to support death in the patient's choice of location.

d) Solve patient issues and problems using knowledge and judgement, throughout the trajectory, and offer suggestions to physicians/nurses on how to manager and anticipate upcoming needs.

e) Participate as an active member of the Hospice Palliative Care Committees and other committees to support the improvement of health system delivery on an ongoing basis as required.

f) Remain current with the latest relevant research and trends.

g) Maintain accurate patient records; update patient charts to document patient progress.

h) Collaborate and consult with community groups and other health care agencies to develop new public education programs and identify and fulfil community health care needs.

i) Prepare documents, records, and reports as required.

a) Personally comply with and assume appropriate supervisory responsibility for compliance with all health and safety practices in the division; in accordance with policies, standard operating procedures/guidelines, and the Occupational Health & Safety Act and applicable regulations.

b) Promote and support a culture of patient safety and advocacy.

Other



a) Comply with all policies and procedures of Listowel-Wingham & Area Family Health Team.

b) Perform all other duties and responsibilities as assigned.

c) Recognize, understand and uphold the FHT's Vision, Mission and Values.

Education/Training/Certifications:



University Degree in Science speciality in Registered Nursing or equivalent. Registered with the College of Nurses of Ontario. Palliative Care nursing education/certification and experience are considered an asset (e.g., Comprehensive Advanced Palliative Care Education).

Experience:



Minimum two years' of related experience.

Skills and Abilities:



Thorough working knowledge of College of Nurses of Ontario principles and standards of practice. Excellent critical thinking and problem-solving skills. Strong interpersonal and customer service skills. Exceptional organizational, change management, problem solving, and verbal and written communication skills. Comprehensive knowledge of palliative care. Ability to adapt to and integrate new technology into practice to support patient care, including working concurrently across multiple technology platforms. Ability to demonstrate tact and discretion in handling confidential and/or sensitive matters; maintain confidentiality. Knowledge of Microsoft Windows and Office applications. Ability to effectively foster good rapport and cooperative working relationships; and to champion the corporate mission and values.

Demands and Working Conditions



Work is typically performed in a standard office setting, as well as some work in the community (home visits). Extensive computer work including, keyboarding, concentration, data entry, and sitting for extended periods of time, is required. Occasional standing and walking for extended periods, depending on the type and format of patient support and treatment. Intermittent requirement to physically assist patients, as required.

The RN Navigator is required to make decisions that require judgement and interpretation. Interaction with clients that may exhibit violent or harassing behaviour, have special needs, or mental illness, and occasional exposure to biohazards, infectious disease, and illness. Continual exposure to human suffering and loss of life.

Job Type: Fixed term contract
Contract length: 3 months

Pay: $37.41-$41.05 per hour

Expected hours: 15 per week
