Registered Nurse-Blood Pressure Nurse

JOB SUMMARY:



The Beamsville Medical Centre Family Health Team is looking for a Registered Nurse to work at a family physician's office. The successful candidate will meet with patients with hypertension, provide counselling regarding the diagnosis and treatment of hypertension, and provide medication advice to patients and physicians to get patients to target. Also, the nurse will do new patient intake interviews as well as provide general nursing coverage for clinic patients meeting with physicians as the schedule demands.

The Registered Nurse provides comprehensive, patient?centered care focused on health promotion, disease prevention, and management of chronic and acute health issues. They work as part of a multidisciplinary team to deliver primary health services to individuals, families, and communities. The RN supports access, continuity, coordination, and quality of care in a primary care setting.

Working Conditions & Schedule

Full time / Part time; may require flexible hours.

Primarily clinic?based.

Participation in team meetings, continuing education activities.

Sometimes evenings or weekends depending on clinic hours or community needs.

Qualifications & Requirements

Registered Nurse with

active, unrestricted registration

with the

College of Nurses of Ontario

(CNO).

Bachelor of Science in Nursing (BScN) preferred; diploma acceptable with relevant experience.

Clinical experience in primary care, community health, or a related setting preferred.

Familiarity with EMR systems and health technologies.

Strong communication, interpersonal, and organizational skills.

Ability to work independently and in multidisciplinary teams.

Knowledge of health promotion / disease prevention, chronic disease management.

Valid CPR / First Aid certification.

EXAMPLES OF DUTIES AND RESPONSIBILITIES:



Key Responsibilities

1.

Assessment & Planning



o Conduct comprehensive health assessments (physical, psychosocial, lifestyle, environmental) of patients across lifespan.

o Identify risk factors, health needs, and potential complications.

o Collaborate with patient, family, and team to develop individualized care plans based on evidence and best practices.

Health Promotion & Disease Prevention



o Promote healthy living through counselling around nutrition, physical activity, smoking cessation, stress management, etc.

o Provide immunizations, screening (e.g. cancer, cardiovascular, diabetes), and other preventive services.

o Educate patients and communities about disease prevention and early recognition of health problems.

Management & Coordination of Care



o Monitor and follow up on chronic conditions (diabetes, hypertension, COPD, etc.).

o Support medication management, treatment plan adherence, and referrals.

o Coordinate care with other providers (physicians, nurse practitioners, specialists, community agencies, etc.).

o Manage patient follow-ups and ensure continuity of care across services/settings.

Client Communication & Education



o Provide education to patients / caregivers about diagnoses, treatments, self-management of conditions, available community resources.

o Facilitate shared decision-making.

o Support behavior change, motivational interviewing as needed.

5.

Documentation & Record Keeping



o Maintain accurate, timely, and confidential patient records using the clinic's EMR (electronic medical record) system.

o Document assessments, care plans, interventions, and outcomes.

o Ensure compliance with privacy legislation and professional standards.

Quality Improvement & Best Practices



o Participate in the evaluation, development, and implementation of best practices and policies.

o Engage in continuous quality improvement initiatives (e.g. audits, patient satisfaction, safety).

o Stay up to date with nursing standards, guidelines (College of Nurses of Ontario), and emerging health evidence.

Team Collaboration & Leadership



o Work collaboratively with interdisciplinary team members (NPs, physicians, social workers, dietitians, etc.).

o Provide leadership in areas of practice (e.g. care of chronic diseases, immunization, health promotion).

o May supervise or delegate tasks to other staff (RPNs, PSWs) as per scope and policy.

Ethical & Legal Compliance



o Practice in accordance with the College of Nurses of Ontario's Standards of Practice, Code of Conduct, applicable legislation (e.g. Regulated Health Professions Act).

o Maintain licensure and professional development, including continuing competence requirements.

o Maintain patient confidentiality and uphold ethical principles in care.

The preferred candidate should have 1-3 years of experience preferably in a community health setting working with interdisciplinary teams, or in a hospital or public health environment. Knowledge and experience in the application of electronic medical records (EMR) TELUS is an asset.
