about the role:
The Sherbourne Health Bus has been serving homeless and underhoused clients in Downtown Toronto for over 30 years, making us a leader in the field of mobile health, with a model that has grown to include primary health care, brief case management and harm reduction.
We are presently looking for a Registered Nurse (RN) to join the Sherbourne Health Bus as an integral part of our mobile health team to deliver primary care, brief case management and harm reduction services to homeless and precariously housed individuals with complex mental health and substance use issues in the Downtown East Community. At Sherbourne Health, our Registered Nurses are key members of the interdisciplinary care team and work to their full scope of practice providing primary care and community health services across the lifespan. Our nurses have taken leadership roles in exciting, multisectoral, partnership-based community health projects, including community vaccination clinics (for influenza and COVID-19), specialized HIV clinics for people without immigration status, low-barrier COVID-19 testing clinics, and community-focused cancer screening initiatives. day to day responsibilities:
Provide primary nursing care, in collaboration with the Health Bus Nurse Practitioner and social supports team, to homeless and underhoused individuals in downtown Toronto.
Conduct health assessments, focused health history taking and triage for episodic or urgent health concerns.
Implement therapeutic interventions for preventative health, health maintenance and health restoration that are suitable to clients facing homelessness and precarity.
Provide health education and health promotion that is responsive to client needs in areas such as preventative care, trans/non-binary health, sexual health, substance use, harm reduction, and infectious and chronic disease.
Offer clinical support onsite to our low barrier drop-ins for homeless and underhoused individuals, including WINK (Women in Need Clinic), STASH (harm reduction drop-in), and our Trans Health Clinic.
Lead primary care appointments on the health bus that include wound care, footcare, venipuncture, STI testing and treatment, brief supportive mental health and crisis counselling, and episodic health assessments.
Facilitate client referrals to appropriate internal and external health and social services, including diagnostic services, support programs, and community-based health agencies.
Play a leadership role in supporting the planning, implementation, and evaluation of community-based initiatives including annual, low-barrier community influenza vaccine clinics.
Participate in quality improvement initiatives and organization-wide committees
To thrive in this role, you'll need
education/experience:
Current Registration with the College of Nurses of Ontario (CNO) in the Registered Nurse - General Class
Baccalaureate degree in nursing (Bachelor of Science in Nursing/BScN) or diploma entry registered nurse
Current Basic Life Support (BLS) certification
Minimum 3 to 5 years demonstrated nursing experience in a community health, primary care, or similar setting.
Valid G-class license and driving record in good standing to operate the Health Bus.
skills and abilities:
Experience working with members of marginalized communities, including those impacted by homelessness, substance use, mental health, poverty, sexual identity, gender identity, and/or residency status.
An understanding of the social determinants of health and harm reduction.
Nursing care knowledge related to primary care, community health, and chronic disease management.
Demonstrated ability to work effectively within a multidisciplinary team.
Solid interpersonal and crisis intervention skills
Effective organizational, critical-thinking, problem-solving, decision-making and communication skills.
Experience and comfort working with an electronic medical record (EMR)
Comfortable using Remote Interpretation Ontario (RIO) phone interpretation line during client assessments.
Proven ability to problem-solve effectively and independently.
Working knowledge of Microsoft Office suite, including Word, PowerPoint, and Microsoft Teams is an asset.
Experience with health program planning, implementation, and evaluation is an asset.
Working knowledge of acts, legislation, and professional standards that govern nursing practice including the Personal Health Information Protection Act (PHIPA), Regulated Health Professions Act (RHPA), and College of Nurses of Ontario (CNO) Standards and Guidelines
Ability to maintain professional practice regarding commitments to colleagues and clients, pertinent legislation, evidence-based standards of care and professional standards of practice.
Ability to speak a second language (including French) is an asset.

working conditions:
The Registered Nurse works primarily on the Sherbourne Mobile Health Bus. Follow up at Sherbourne Health or other sites in the community when applicable. This role will require work primarily during daytime hours but may require occasional evenings and weekends.
