Job Summary
The Registered Nurse IIC (RN IIC) will work with an inter-professional team in a collaborative team environment to provide access and continuity of care to improve health outcomes and patient quality of life, with the support and wrap around services of the full team. The Registered Nurse II is a leadership role responsible for coordination of clinical patient care as well as assisting patients and their families to navigate family care teams and community services. The Registered Nurse IIC will practice to their full scope to meet patient needs; participate in direct care of patients and provide clinical leadership, including:
Comprehensive assessment, preventative screening, healthy lifestyle support, education, health coaching, motivational interviewing, and chronic disease case management of individuals and families of all ages, incorporating the Determinants of Health;
Support individuals and their families to achieve optimal health and well-being while fostering self-management;
Utilizing a Person and Family Centered approach, identifies personal goals of care and uses the nursing process to plan, develop, implement and evaluate individualized plans of care based on established current best clinical practices, in collaboration with the inter-professional team;
Lead others in the BETTER program approach, for chronic disease screening, prevention and management
Lead/participate in ongoing development, implementation and evaluation of comprehensive education programs for individuals with chronic diseases as well as their support systems;
Facilitation of integrated Virtual Care options, including remote patient monitoring for chronic disease screening, prevention and management;
Recognizing cultural diversity and practicing culturally safe care;
Practicing age-friendly care in a collaborative care team setting;
Work closely with the manager and other team members in program development, and patient and clinician engagement;
Participate in evaluation and continuous quality improvement processes based upon current best practice guidelines;
Required Qualifications:
Graduation from an approved/recognized school of nursing program is required;
Current and continuing practicing licensing with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required;
A minimum of 5 years of experience within the last 7 years working in an adult Acute Care medicine/surgical setting, primary health care setting, or community clinic setting (Combinations of education and experience will be considered);
Current BLS certification;
Demonstrated progressive leadership, organizational, interpersonal and decision-making skills;
Demonstrated effective oral and written communication skills;
Demonstrated strong collaboration skills and the ability to create and maintain successful working relationships;
Ability to self-direct and work independently and collaboration with clinic team members;
Proficiency with TELUS Med Access EMR, Meditech, CRMS, and Microsoft Office tools;
A valid Newfoundland and Labrador Class 5 Drivers license the ability to travel and use of a private vehicle is required;
The successful applicant will be required to submit a satisfactory certificate of conduct and Vulnerable Sector Check;
Additional Qualifications/Assets:
Experience in the care and management of patients with diabetes, COPD, heart failure, and/or mental health disorders;
Successful completion of an approved course in chronic disease management (BETTER; Improving Health My Way) and/or Canadian Diabetes Educator (CDE) certification an asset.
Previous experience in adult education;
Knowledge in change leadership and quality improvement methodologies;
Experience working in a collaborative team model;
Experience working in and understanding of the unique challenges and opportunities of rural health care services delivery;
