FCT's will enable the delivery of a range of quality care for clients within their own communities while facilitating interdisciplinary work amongst healthcare providers.

The Registered Nurse IIA will conduct advanced client assessment using Electronic Medical Record (EMR) documentation and will lead others in the BETTER program approach and remote patient monitoring for chronic disease screening, management and prevention approaches including education, consultation, motivational interviewing, detailed assessment, investigation, follow-up, and evaluation within their individual and clinic case load. This position also involves the ongoing development, implementation and evaluation of comprehensive education programs for individuals with chronic diseases as well as their support systems. The nurse will work closely with the manager and other team members in program development, and client and clinician engagement. The role will encompass participating in clinic evaluations and continuous quality improvement processes based upon current best practice guidelines. Flexible hours are required (combination of days, evenings, and weekends required). Travel throughout the region may be required.
The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.
Required qualifications:
Undergraduate Degree in Nursing.
Current registration with the College of Registered Nurses of Newfoundland and Labrador.
A minimum of 3 years of experience within the last 5 years working in an adult acute care medicine/surgical setting, primary health care setting, or community nursing program
Current BLS certification for health care providers from the Heart and Stroke Foundation.
Current Health Assessment certification or willingness to complete the next available course.
Demonstrated exceptional leadership qualities and problem-solving skills, as well as, oral and written communication skills.
Demonstrated ability to work well independently and collaboratively with others in an interdisciplinary team environment and community.
Demonstrated ability to be self-directed and exercise independent nursing judgment in a community-based environment with limited supervision.
Demonstrated initiative.
Demonstrated ability to develop, implement, and evaluate community health nursing programs and healthy public policy in a population health framework.
Demonstrated ability to apply basic computer skills.
Willing to work a flexible work schedule.
A valid Newfoundland and Labrador Class V Driver's License.
Consistent demonstration of Central Health's corporate values; Compassion, Accountability, Respect, Stewardship and Quality
A satisfactory record of work performance and attendance is required for this position.
A satisfactory certificate of conduct from the RCMP or local police authority.
Demonstrated skill in using health care-based computer systems (CRMS, Meditech, EMR).
CNA certification in Community Health Nursing.
Experience in the care and management of patients with diabetes, COPD, heart failure, and/or mental health disorders.
Successful completion of an approved chronic disease management course.
Successful completion of the Canadian Diabetes Educator (CDE) certification.
Training as a BETTER practitioner.
Previous experience in adult education.
