Job Summary
The Chronic Disease Prevention and Management Registered Nurse II is a leadership role responsible for the coordination and management of patients living with chronic disease through Intensive Case Management (ICM). The role will use critical thinking and clinical judgement in coordinating, facilitating, and improving the management of patients living with chronic disease within an integrated primary health care model of care. The Chronic Disease Prevention and Management Registered Nurse II role provides comprehensive and coordinated care to patients as a member of an interdisciplinary primary health care team. The role is responsible for accessing, consulting and mobilizing available team and community resources to assist patients in maintaining and achieving optimal health through collaborative goal setting, action planning, ICM, education, health promotion, and treatment to facilitate goal achievement. The role further supports preventative screening through utilizing the BETTER approach which focuses on prevention and screening of cancer, diabetes, heart disease and associated lifestyle factors (nutrition, physical activity, smoking, and alcohol consumption).
The Chronic Disease Prevention and Management Registered Nurse II participates as a member of a multidisciplinary team in the planning, implementation, development, and evaluation of individual care plans for patients living with chronic disease. The role supports patient self-management of the chronic disease through teaching and counselling, on a group and individual basis, in order to promote and assist with health lifestyle adjustments; conducts outpatient assessments, shares information and collaborates with other health care team members in order to ensure the provision of quality chronic disease management services through the health care system. The Nurse II is a champion for identifying and referring patients to existing Chronic Disease programs such as the Comprehensive Respiratory Care Program. Travel throughout the region may be required.

Occasionally moves or lifts equipment weighing between 10-25 lbs.
Occasionally may have to assist patients such as push or pull patients over 50 lbs. in a wheelchair or help them stand on a weight scale.
May require constant sitting to counsel patients and family members, perform clinical procedures, demonstrate equipment handling, and document information on the computer.
Occasional walking or standing to deliver presentations, conduct clinics and consult patients
Regularly performs fine finger/precision work for machine operation and computer documentation.
Visual concentration is required to review and interpret patient test results, give medication, and observe patient\'s condition.
Auditory concentration is required to listen to patients and family members during counselling.
Other sensory demands such as touch are required to check patients for changes in temperature, palpate pulses, perform tests/procedures such as blood pressure or vital signs.
Job Qualifications
Graduation from an approved/recognized school of nursing program is required.
Current and continuing practicing license with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) is required.
A minimum of 5 years of experience within the last 7 years working in an adult acute care medicine/surgical setting, primary health care setting, or community clinic setting is required.
Experience in the development, implementation and evaluation of community programming is considered an asset.
Current in BLS.
Proficient in computer programs/software including Electronic Medical Record (EMR) and Microsoft Office tools.
Demonstrated knowledge of current standards for chronic disease management.
Demonstrated understanding of the principle strategies around empowerment and motivation.
Ability to communicate effectively, both verbally and in writing.
Ability to work independently and as a member of a multidisciplinary team.
Demonstrated ability to teach and implement a chronic disease education program for patients/clients/families, in both an individual and group setting.
Ability to create and maintain rapport with the patient/client/family and with other health care providers.
A valid class five driver\'s license and use of a private vehicle are conditions of employment.
A satisfactory record of work performance and attendance is required.

- Intensive Case Management (ICM) training
- Electronic Medical Record (EMR) training
