If you like variety and thrive on giving client and family-centered care within an interdisciplinary healthcare team, this Home Care Case Manager position will appeal to you. In this regularized relief position, you will work autonomously in both community and facility as department needs require. Involving client and family support networks, you will work with clients across the age spectrum, from birth to end of life. The autonomous nature of this role requires you to have strong assessment, critical thinking, and decision-making skills. Evening, weekend, night and after hours on call work with limited staff (only one case manager on call) is required. When on call, you may be required to go to community or facility settings to address client issues. You must have a reliable vehicle with business insurance and be comfortable driving in the dark, in variable weather conditions and in rural areas. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0) Additional Required Qualifications:

3-5 years RN experience dealing with entire age spectrum including seniors. At least one year of current Canadian nursing experience. Demonstrated skills in problem-solving, case management, interpersonal relationships, and organizational leadership. Excellent ability in the organization and delivery of care to senior\'s health clients. Ability to work in a fast-paced environment; ability to be flexible to time commitments and to meet the needs of clients in the Senior\'s Health program. Preferred Qualifications:

Previous experience in a Home Care role. RAI-HC certification. Supportive Pathways/P.I.E.C.E.S certification. Palliative Care course/experience. Wound care course/experience. It\'s Your Move course. Medication Assistance Program (MAP).
