POSITION SUMMARY:
The Emergency Department Admission Avoidance Program seeks to identify, strategize and implement discharge planning as soon as a patient arrives at St Joseph's Healthcare Hamilton. We strive for excellence, family involvement, and patient satisfaction through optimal outcomes for all involved. Admission Avoidance was adopted in 2016 out of great need to better serve the community demand for Emergency Care
The Admission Avoidance team allocates in-house, community and provincially funded resources to get patients home sooner and straight from the Emergency Department

The Admission Avoidance team includes Registered Nurses, Physiotherapists, Occupational Therapists, Social Workers, Community Care, Coordinators, Physicians, Families and Caregivers
The Registered Nurse (RN) is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the SJHH mission, vision and values, policies and procedures; and applicable specialty standards. The RN provides patient/client-centered and evidence informed care to promote health and assess the need for the provision of care and the treatment of health conditions. The RN collaborates with the inter-professional team and assumes a leadership role in the delivering a full range of care and treatment to patient/clients including those with complex needs, high acuity and less predictable outcomes.
QUALIFICATIONS:

  • Current registration with the College of Nurses of Ontario required.
  • Strong clinical assessment skills required.
  • Demonstrated ability to utilize principles of knowledge transfer to facilitate practice change & implementation of best practices
  • Current BCLS/HCP certification required.
  • Experience working with geriatric patients an asset
  • Gentle Persuasive Approach training preferred.
  • CPI - Crisis Prevention Training (or successful completion as part of clinical orientation) required.
  • Comprehensive knowledge of community resources and health care delivery systems in acute and community settings.
  • Demonstrated knowledge, sensitivity and expertise in the care of patients and their families.
  • Demonstrated ability to respond to clients and situations with flexibility and adaptability.
  • Demonstrated knowledge and practice of safe and secure environment for clients/families/teams by listening, discussing, exploring, clarifying and acting on their concerns, needs and priorities required.
  • Demonstrated expertise and competence in areas of client need, including: chronic disease management, mental health and addictions, complex and dysfunctional family systems, older adults who are frail and/or palliative, medication and symptom management for specific diseases.
  • Excellent interpersonal, communication, organizational and decision-making skills required.
  • Demonstrated leadership skills, including problem solving, critical thinking, conflict resolution and negotiation .
  • Demonstrated ability to work independently and collaboratively with an interdisciplinary team, family members, support networks, and a variety of formal and informal service providers.
  • Excellent time management and stress management skills required.
  • Demonstrated knowledge of and experience in the application of adult education principles preferred.
  • Demonstrated knowledge and understanding of P.I.E.C.E.S model of assessment and ability to apply preferred.
  • Knowledge of the Occupational Health and Safety Act and Regulations, professional standards and guidelines, SJHH corporate and departmental Policies and Procedures, including successful completion of WHMIS training required.
  • Knowledge of documentation standards, privacy legislation, and health record management processes required.
  • Computer literacy and basic proficiency is required.
RESPONSIBILITIES:
  • Provides direct patient care for patients.
  • Demonstrates proper care of patients and patient care equipment.
  • Communicates effectively with all staff, including the inter-professional team, family members, support network, community partners, and a variety of informal and formal service providers.
  • Communicates with peers regarding any changes or needs throughout the work day.
  • Works as a team member to complete required work in the area for the day.
  • Follows infection control guidelines during patient care.
  • Ensures an efficient and effective working environment with timely patient care.
  • Works with management and multidisciplinary team to implement best practice and organizational mandates.
  • Supports the completion of appropriate community and program referrals.
  • Responds to changes in condition and functional decline to support palliative and end-of-life care.
