The Geriatric Mental Health Services at CAMH is seeking a full-time, permanent, Registered Nurse to join the Hospital to Home (H2H) Program. As part of this new and evolving program, you will apply strong critical thinking and problem-solving skills to support appropriate client intake into H2H. Your responsibilities will include conducting comprehensive assessments including history taking and collateral gathering, liaising with and travelling to the CAMH Emergency Department and other CAMH programs, and working collaboratively to develop, implement, and evaluate inter-professional care plans. You will provide care to older adults with complex mental health and addiction concerns, often complicated by behavioral challenges and physical health issues. In addition, you will work closely with colleagues across Geriatric Mental Health Services and with community partners to ensure coordinated care. You will also contribute to program development and demonstrate flexibility as the H2H program continues to grow and establish itself. You will practice in accordance with the College of Nurses of Ontario professional standards, the Canadian Standards for Psychiatric-Mental Health Nursing and Gerontology, CAMH policies and procedures, and the CAMH Practice Model of Care. Your role will include participating in clinical supervision, providing education, mentorship, and supervision to students, and contributing to research and evaluation initiatives. You will also support a healthy workplace that values diversity, fosters teamwork, and complies with all legislative and regulatory requirements. This position requires weekday hours.


Job Requirements




The successful candidate must be a registered member in good standing with the College of Nurses of Ontario and hold current B.C.L.S. (CPR) certification. Preference will be given to candidates who possess a a B.Sc.N, and possess/or are working toward CNA certification in Psychiatric/Mental Health Nursing or Gerontology. You must demonstrate strong communication and interpersonal skills to support therapeutic nurse-client relationships, provide family-centered care, and collaborate effectively within inter-professional teams and different community partners. You should have the ability to perform comprehensive assessments, triage multiple clients' needs, monitor responses to pharmacological and non-pharmacological interventions, and communicate effectively with team members. A solid knowledge base in gerontology, geriatric mental health nursing, addictions, crisis intervention, trauma-informed care, and behavioral support strategies is required. Familiarity with normal aging processes, geriatric treatment modalities, community resources, and issues related to addictions in older adults is required. Experience in mental health nursing with seniors living with mental illness and/or addictions is an asset. The ability to work effectively with clients from diverse cultural and social backgrounds is essential. Computer literacy is required.
