The Registered Nurse (RN) Case Manager collaborates, coordinates, and delivers healthcare services, with expertise in advocacy and a patient- and family-centered approach. You will work with clients across the life span with various needs in the home setting. You must be willing to drive up to one hour from your home site to the clients' residence during all seasons and at various times of the day. This position does have some physical requirements such as bending, lifting, twisting, walking, climbing stairs, and carrying. The lifting and carrying could include heavy items. This position requires on call duties. Responsibilities include assessing, planning, implementing, and evaluating client care, focusing on chronic disease management, health assessments, wound care, ADL support, placement in facilities, and palliative care. The RN Case Manager collaborates with individuals, caregivers, and community resources to support healthy living and community connectedness. Your strong interpersonal skills will be used daily as will your strong documentation capabilities. Located in beautiful Fort McMurray which resides on Treaty 8 territory, and on the traditional lands of the Metis. Each Indigenous community is diverse in size, culture, and character. As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
