The Rehabilitation Navigator navigates patients to timely access of inpatient rehabilitation at the Glenrose Rehabilitation Hospital and plays an integral role in coordinating discharge plans. This highly complex role includes receiving and assessing incoming consults and referrals while having direct contact with referring partners, collaborating with various Glenrose multidisciplinary team members, and utilizing information sources. Once it is confirmed that the referred patient meets admission criteria for inpatient rehabilitation, admissions are coordinated with upcoming discharges. This requires critical thinking and accurate interpretation of complex information as well as a comprehensive understanding of waitlists, the impact of patient flow across the Edmonton zone and the collaboration with community partners and the Glenrose multidisciplinary team members to prioritize admissions accordingly. As an integral member of the multidisciplinary team, the Rehabilitation Navigator plays a key role in team and family conferences and ensuring that discharge planning remains on track. The Rehabilitation Navigators must complete appropriate referrals to other programs/services, appropriate documentation, process and outcome evaluation and professional development activities. Major responsibilities of the Rehabilitation Navigator include completing comprehensive electronic assessments, determining priority, implementing appropriate services plans, liaising with continuing care, IOC, and RAAPID, and providing information and support to clients, families, and caregivers to facilitate safe discharges efficiently and effectively.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.
