Your Opportunity:
Reporting to the Manager, Chronic Disease Management, the Registered Nurse will work as part of a multi-disciplinary team within Primary Care and Chronic Disease Management (PCCDM), applying expertise in obesity, diabetes and self-management. Responsibilities include participating in the development, provision, and evaluation of group education sessions for people with chronic conditions. The Registered Nurse will be responsible for the provision of education, advice, and information using evidence-based guidelines for chronic disease prevention and management for adults. They will be responsible for the assessment and planning of client referrals to specialty care. In addition, they will facilitate linkages with positive relationships with local community and ambulatory program resources both internal and external to Primary Care Alberta (PCA). The Registered Nurse will be a resource for clients, physicians, and other health care professionals in PCA and other community health and social services agencies, in relation to all aspects of chronic disease management and self-management. They are required to work independently outside regular working hours in unique community settings and with diverse and vulnerable populations, as well as online and on telephone. This position is in the PCA and thus in the event of a public health emergency, staff from this program will be deployed as required. This position will be part of Primary Care Alberta effective November 18, 2024. As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Psychiatric Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): https://connect.nurses.ab.ca/home/learning-and-development/learning-modules NextGenU Health Assessment Resources Certification: https://courses.nextgenu.org/course/view.php?id=271.

Additional Required Qualifications:

Clinical experience in supporting community-based clients with chronic disease management, prevention, and self-management strategies in community health settings program context. (minimum 3 years' experience with metabolic illnesses such as Diabetes, Obesity and Heart Conditions). Understands the importance of community engagement and partnerships. Minimum 2 years' experience educating health care professionals re: management of Obesity and 2 years of recent experience with Diabetes management. Minimum 3 years of recent experience in the delivery of group patient and provider education in person and online. Experience working with individuals and groups in community, using behaviour change models and applying strategies for change. Ability to lift, handle and carry up to 30 pounds of equipment. The role may require continued use of personal protective equipment. Must be able to raise arms above head to appropriately don and doff PPE as well as lift equipment and supplies. Requires sitting and/or standing for long periods of time. Requires walking several blocks and climbing several flights of stairs. Requires packing, lifting, and transporting equipment to and from community sites and travel within the Edmonton and surrounding area.

Preferred Qualifications:

Baccalaureate Degree in Nursing CDE (Certified Diabetes Educator) / CBE (Certified Bariatric Educator) designation. Experience working with diverse and vulnerable populations. Completion of a recognized course in adult learning. Experience working with Connect Care (Ambulatory and Community).
