Your Opportunity:
As an experienced Oncology Registered Nurse you will make a difference as the new Head and Neck Cancer Patient Navigator (H&N CPN), Cancer Care Alberta (CCA) supporting head and neck cancer patients at Cross Cancer Institute (CCI). H&N CPN will join our provincial navigation team to focus on enhancing continuity of care for H&N cancer treatment trajectory from surgery to ambulatory cancer treatments (radiation, systemic therapy treatment, rehabilitation, follow-up, post-treatment), into survivorship or palliative care/end of life care with a focus on care coordination at key transition points. In collaboration with health care teams, provides targeted symptom assessment and management including identifying patients\' symptoms and concerns, providing resources/supports about their disease and care plan, possible treatments, side effects, and making referrals to other programs as required. Provide tailored interventions and services to address physical, psychological, social, emotional, spiritual, and/or practical needs via in-person and virtual formats. Collaboration with key stakeholders: Supportive care (rehab/psychosocial), Community Providers (e.g. Family Physicians/Dental), Surgery, HC, Hospice, IP Units, Medical/Radiation Oncology, Palliative Care, patient reps, Wellspring, Recovery Alberta and others. Support through education/health promotion by helping understand test results, next steps in care, identifying care gaps and developing strategies to mitigate gaps, and by providing patient/family centered education and promoting self-management, informed decision making, goals of care and transitional support within CCA. Referrals from Community Providers (e.g. Family Physicians and Dental Practitioners), the CCI H&N triage coordinators, head and neck clinics located at the University of Alberta, and other clinics/routes.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
