We are looking to expand our Primary Health Care team with the addition of a new Registered Nurse.
Position Statement/Summary
As a member of the inter-professional primary health care team, the Registered Nurse provides primary health care to individuals, families and the community, and participates in the development and implementation of quality improvement initiatives with a focus on excellence and coordination of care. This position is designed to provide integrated community health services for newcomer and low-income families with children ages 2-6 years. There are opportunities to support and/or lead care teams in terms of quality and performance improvement. This is a unionized position.
Key Responsibilities
Performs physical assessments, treatment, triage, health education, and other clinical activities according to the College of Nurses of Ontario standards and full scope of nursing practice.
Provides comprehensive primary medical care to populations facing multiple barriers, ensuring access to those most vulnerable, focusing on pre and post-partum wellness, well-baby, well-child and breast/chest feeding.
Provides primary care to children 2-6 years and their families within a health care modality that respects their needs and choices.
Works in partnership with other team members to plan, provide and coordinate care, including psychiatry, primary care and allied health care providers.
Provides Well-Child Clinics in the community, at designated sites, within a framework that emphasizes health promotion, disease prevention and early identification and intervention of children's developmental and learning needs.
Provide itinerant services working from different locations to create greater access to populations facing barriers to care.
Identifies and/or develops educational resources to support health promotion and health education activities.
Develops, facilitates and evaluates clinical programs in partnership.
Maintains complete, timely and accurate client records, assessment notes and required correspondence.
Supports the development, implementation, monitoring and evaluation of treatment, education, screening and health promotion programs for individuals, families and the community to foster self-help, mutual aid, and capacity building and address needs of the community.
Participates in quality improvement projects to improve client care, access to care and work processes.
Where appropriate, develops partnerships and alliances to optimize health care service provision and address needs of community.
Contributes to programs and services cycle of planning and evaluation, and participates in centre-wide planning, committees, and team meetings as appropriate.
Participates in inter-professional research with colleagues - internal and external to the Centre, as appropriate.
Maintains all licenses, certificates, and standards related to the ability to practice as an RN in Ontario.
Perform duties as assigned by the supervisor.
Qualifications
An undergraduate degree in nursing from a CNO-approved university or college program is required.
Must be registered and in good standing as a Registered Nurse with the College of Nurses of Ontario.
Must have the ability to work to the full scope of practice for a Registered Nurse.
Must demonstrate thorough knowledge and proficiency in current nursing physical assessment and treatment methods.
Candidates with previous nursing experience and community nursing experience, in a primary care setting, are preferred.
Familiarity and understanding of Population Health and Equity in Health Care including an understanding of Anti-Racism and appreciation of Anti-Racism in the Ontario and Canadian context, and in particular Anti-Black Racism and Anti-Indigenous Racism.
Must have experience working with pre-post natal individuals and children aged 2-6 years.
Training or certification in breast/chest feeding, an asset.
Experience with program development, implementation, monitoring and evaluation, an asset.
Experience in a community setting or combination of community and/or public health setting.
Experience providing comprehensive primary medical care to populations facing multiple barriers and with comorbidities, including mental health and addictions.
Demonstrated ability, commitment to and knowledge of community health care.
Demonstrated ability to work in a team-based model and within an inter-professional team environment.
Demonstrated leadership skills.
Proficiency in the use of computers, various software applications and electronic health records.
Excellent interpersonal skills.
Excellent communication, decision-making, problem-solving, conflict management and time management skills.
Ability to speak a second language, relevant to catchment area populations (Spanish, Portuguese, Polish, Ukrainian, Russian, Arabic, Tigrinya), is an asset.
