POSITION SUMMARY:



The successful candidate will work as a member of an interdisciplinary team providing outreach, consultation and transitional supports to individuals with dual diagnosis (psychiatric and developmental disabilities), care providers and families.

QUALIFICATIONS:



Current registration with the College of Nurses of Ontario required CNA certification in Psychiatric/Mental Health Nursing (CPMHN(c)) preferred Minimum of 3 years experience working with individuals with dual diagnosis Demonstrated experience, knowledge and skills in working with individuals with both serious mental illness and developmental disability. This should include: knowledge of best practices in working with individuals with dual diagnosis; knowledge of the common syndromes associated with developmental disability; knowledge of the complexities related to having both a psychiatric disability and developmental disability; knowledge of commonly used medications for this population, their purpose, side effects and interactions; knowledge of basic behaviour therapy principles and techniques. Knowledge of the Mental Health Act, CNO Standards of Practice, Hospital and Departmental policies and guidelines Demonstrated ability to consult and work with community care partners and inpatient program staff Demonstrated ability to create a client?centered plan of care, discharge and follow?up plan specific to this specialized population Excellent knowledge of the related healthcare, housing etc.. services for individuals with dual diagnosis in Hamilton and in all of St. Joseph's Healthcare service regions Demonstrated knowledge and understanding of principles of Psychosocial Rehabilitation (PSR) Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families Gentle Persuasive Approach training preferred Demonstrated knowledge and skill in the application of nursing process & nursing diagnosis Demonstrated ability to develop and maintain effective professional and therapeutic relationships with caregivers, other professionals and family members Demonstrated ability to teach others including those who are not medically trained Willingness to participate in ongoing learning Ability to work independently, make emergent decisions, and problem solve Demonstrated ability to work well in an interdisciplinary environment Current BCLS/HCP Certification required Must have valid driver's license and access to a vehicle Good attendance and work record is essential
