Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. If you are interested in assisting client's with acute and chronic conditions to maintain their independence in the community, this is position may interest you. We are looking for a Registered Nurse to join our Community Aides to Daily Living/Continuing Care Access team. Using your knowledge of the Albert aids to Daily Living (AADL) program, you will screen clients to determine if they are eligible to benefit from supplies and equipment provided through the AADL program. Using strong tele-triage assessment skills, you will ensure client's receive appropriate medical/surgical supplies through the AADL program, or if the client's needs would best be managed through a home care or supportive living program. As a Registered Nurse you will provide a wide variety of nursing services to patients, families, communities and populations. Using strong tele-triage skills, you will assess, identify and clarify the needs of the client and triage referrals to the appropriate agencies and services partners. Part of these responsibilities include current knowledge of AADL eligibility, assessment of needs, utilizing nursing processes through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. This position will be located at Seventh Street Plaza with CCA.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Additional Required Qualifications:

Excellent verbal, written and telephone communication skills are necessary as well as experience tele triaging complex clients and situations. Experience with Connect Care Communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment. Excellent interpersonal skills. Proven critical thinking and decision-making skills. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet. Experience with RAI Home Care assessment tools. Experience with complex clients including acute, chronic and end of life conditions.

Preferred Qualifications:

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Two years current nursing practice with one year in a Continuing Care based setting. Experience with advanced telecommunications systems would be a benefit. Minimum of 1-2 years of recent community nursing (i.e. Home Living, Supportive Living) experience is preferred. Current AADL Authorizer status is preferred.
