Job Summary
Reporting to the Manager of Outpatient Clinics/Systemic Therapy Suite Cancer Programs, the Complex Malignant Hematology Registered Nurse will be the primary coordinator for patient care that is delivered both within the CMH program and in collaboration with shared care, leukemia and Stem Cell Transplant teams at Princess Margaret (PM) Cancer Centre.
The CMH Registered Nurse will facilitate communication between the CMH/malignant hematology team, inpatient Cancer Care Unit (CAN), consultants, and other Southlake departments. Enabling and role modeling teamwork, communication, autonomous clinical practice and a drive towards continuous program and team improvement, the individual will work alongside the interprofessional team to optimize the patient experience, improve patient flow and achieve quality patient outcomes.
This position will collaborate with internal and external stakeholders to coordinate patient care as well as develop, implement and sustain best practices to improve quality of care and efficiencies in the CMH program.
Skills & Abilities
Three (3) years current or recent malignant hematology oncology experience required.
Recent systemic therapy/chemotherapy/biotherapy administration experience an asset.
Extensive knowledge of malignant hematology disease and treatment trajectories.
1-2 years leadership experience or experience working in an advanced clinical practice role preferred.
Previous project management experience working in an advanced clinical practice role preferred.
Demonstrated ability to communicate effectively.
Excellent verbal and written communication skills.
Strong interprofessional and time management skills.
Excellent teaching skills with the ability to teach one-on-one and/or groups.

Qualifications
Current Registration with the College of Nurses of Ontario or Current registration with other applicable regulated health professional College of Ontario.
Current BCLS, Level II Rescuer certification required.
Canadian Oncology Nurse Certificate required
Membership to Oncology related association considered an asset.
Successful completion of de Souza Complex Malignant Hematology required
Baccalaureate Degree in a health-related field preferred.

As an integral member of the Malignant Hematology Team, the responsibilities of the Registered Nurse, Complex Malignant Haematology include:
Utilizing and applying current specialized knowledge and expertise and recent advances in the field of Complex Malignant Hematology to facilitate the effective management of clinical activities.
This position involves the coordination of in/out-patient care and continuity in patient care management
Performing patient care responsibilities
Performing teaching/education responsibilities
Participating actively in Quarterly Improvement initiatives
Participating in academic and research activities/projects
Performing cross-functional and/or other duties consistent with the job classification, as assigned or requested.
Contributes to the professional and patient population learning and education by:
Providing leadership to educational activities/programs.
Facilitating and providing educational opportunities and programs, in collaboration with the Nurse Educator, for nursing, medical and allied health students and staff for the enhancement of specialized knowledge and skills related to the specialty area.
Mentor/Preceptor of students (graduate and undergraduate) and other interdisciplinary students as required in their clinical experience with evaluation of performance.
Engaging in joint activities with Faculties of Nursing, and other academic faculties and organizations through collaborative projects, research and cross appointments.
Utilizes advanced preparation and knowledge of patient population needs, processes of patient care, care systems, the research process and interprofessional collaboration to provide and support leadership in the following areas:
The development, implementation and evaluation of quality management programs / initiatives, care delivery models, practice standard / guidelines / policies and procedures and associated health outcomes and resource utilization, in collaboration with the leadership teams.
Identify patterns and trends in health care and patient care outcomes and develop alternate approaches in relation to patient care situations.
Participates on committees relevant to patient care issues.
Promotes the integration of advanced nursing practice in program planning, policy making and problem solving.
Performs other duties consistent with the job classification, as required.
