Within the standards of nursing practice established by the College of Nurses, will provide medical care and ongoing support to all Home & Community Care clients and seniors in the Extended Care Unit (as required) and ensure their physical, mental, emotional and spiritual needs are being met. Assist in the formation of a formal palliative care framework.
QUALIFICATIONS
Education:
University Degree. Registered and in good standing with CNO and RNAO.
Skills and Abilities:
Must have First Aid/CPR.
Training in Palliative Care, Dementia, Alzheimer's, Gerontology and Diabetes.
Excellent communication and interpersonal skills.
Demonstrated leadership skills with ability to effectively act as lead on shift coordinating client care.
Sensitivity and understanding of Indigenous issues and provide a trauma informed approach to health care.
Anishnaabemowin an asset.
Experience:
1 to 3 years in long term care/working with seniors.
Experience working with Indigenous families and communities, knowledge of Indigenous culture, history and detriments of health.
Experience in health care administration, Palliative Care, Wound Care (as qualified).
Other: A Vulnerable Sector Screening is required for this position.

Home & Community Care (HCC):
Act as a lead and deliver direct nursing care including perform assessments, provide client care & recovery plans, conduct home care visits, and manage Pharmacy/medications for all HCC clients as required.
Monitor clients conditions, assess their ability to function independently and communicate client updates to HCC Health Care Team daily.
Coordinate nursing care services with the HCC client, physician, hospital, rehabilitation and other health care professionals under the supervision of the manager.
Conduct, chart/document all assessments, observations, physician's orders, update care plan and care provided for all HCC clients.
Provide nursing education/support to clients & family, PSWs, Homemakers and Health Navigator.
Collaborate with members of health care team to ensure efficient, consistent and comprehensive HCC/ECU care planning and delivery.
Evaluate the appropriateness, effectiveness and efficiency of interventions of nursing care designed to meet client needs and adapt as necessary in collaboration with the client, family and health care team as necessary.
Assist with the development of a community specific palliative care program.
Extended Care Unit (ECU):
Work shifts in ECU as needed.
Coordinate and monitor care plans and ensure quality of care for ECU residents.
Conduct, chart/document all assessments, observations, physician's orders, update care plan and care provided for all HCC clients.
Administration and management of medications, nursing procedures & treatments, wound management, monitoring for therapeutic interventions, etc.
Contact Physician and other health care providers as required.
Monitor activities of clients to keep them mobile and as physically and mentally fit as possible.
Coordinate work of PSWs on shift.
Assist with departmental orientation of new employees and in-service education in the ECU.
Promote a healthy environment with clients, staff and visitors at the ECU.
Conduct daily check in and report to next shift.
Maintain open lines of communication among staff, supervisor and families.
Maintain a system of internal control and management reporting.
Receive and replace all shifts for call-ins at the ECU.
