Position Summary:
Support patients within the Clozapine Clinic in partnership with treatment teams within the mental health program
Complete blood draws and collection
Monitor patients' lab values/results
Communicate with patients and treatment teams in event of alert for abnormal lab values and provide information and supports for required follow-up monitoring/treatment
Complete mental health assessments
Complete risk assessments
Frequent collaboration with GenCan Network
Collaborate with community agencies (laboratory facilities, pharmacies) to maintain coordination in the patient's treatment process
Communicate with in-patient care teams for seamless continuation of treatment plans in the outpatient setting upon discharge
Organize and maintain accurate scheduling of appropriate frequency of blood work for patients at the clinic
Provide ongoing education and supports for patients and families
Monitor for and process referrals to the Clozapine clinic
Key Qualifications:
Registered Nurse with current registration with the College of Nurses of Ontario and in good standing
Graduate of a recognized College or School of Nursing, with Baccalaureate degree preferred
CNA certification in psychiatric/mental health nursing preferred
Minimum 2 years current Mental Health experience, including solid knowledge related to working with adult patients with complex psychiatric disorders
Sound working knowledge of legislation related to mental health care and treatment
Expertise and experience in venipuncture
Knowledge of all relevant medications and side effects
Knowledge and experience related to concurrent disorders
Experience with case co-ordination and counselling
Knowledge of Recovery Model
Proven ability to interact with patients and their families in an empathetic manner
Must possess exceptional organizational, time management and critical thinking skills
Excellent assessment and documentation skills
Experience working effectively within an inter-professional collaboration model
Excellent interpersonal, verbal and written communication
Knowledge of community resources
Positive work and attendance record
