A member of the multidisciplinary team the RN (nurse case manager) provides care to patients with stages 4 and 5 Chronic Kidney Disease (CKD). The nurse case manager is responsible for the assessment, planning, implementation and evaluation of patient care. The RN uses nursing knowledge in addition to treatment guidelines to develop a patient care plan using a case management model. The RN reviews lab results, symptoms and other diagnostics to assess the patient's response to care and communicates the results with the nephrologist. The RN provides education to patients including a developing a learning assessment based on past health history, current health status, learning needs, self-care ability and values and beliefs around health care issues and treatment. The RN provides formal education for patients and families on the subject of Renal Replacement Therapy and Conservative Kidney Management. The RN also participates in program development and quality improvement initiatives. This position is based in Grande Prairie, and it is eligible for the remote retention allowance. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
