POSITION SUMMARY



Under the direction of the Director, Health and Community Services, the RN - CDE is responsible for providing clinical support for primary care, and diabetes support and education for clients, their families and other healthcare professionals. This position provides primary care services as part of an interprofessional team, with a focus on supporting clients living with, or preventing diabetes. Primary care services will include general clinical duties as needed, as well as providing information to clients' Primary Care Provider on options available for clients' treatment, identifying the needs, and implementing an individualized care program pertaining to diabetes. The emphasis is on a holistic approach to chronic disease management, health promotion, disease prevention, and to support clients and their family in a culturally appropriate manner. The Registered Nurse will practice under the Standards of the College of Nurses of Ontario, and the Philosophy and Values of Nursing Services including all policies of Anishnawbe Mushkiki Community Health and Wellness.

DUTIES



Will provide general clinical support to Primary Care Providers, and other nursing tasks as needed, within full scope of practice. Will work within current medical directives to provide care for clients to manage their diabetes. As a Registered Nurse, acts as a resource for diabetes clients, their families and other health professionals and partner agencies of Anishnawbe Mushkiki. Assesses clients' treatment and educational needs on an ongoing basis. Partners with clients to assist with decisions regarding their diabetes and management as well as supporting clients in healthy eating active living changes. Assesses clients to determine if community referrals are required for the client due to their care needs and or educational needs. Participates in program planning and resource development initiatives. Duties include communicating effectively with the referring physician and other healthcare professionals regarding clients' needs, care and progress. Maintain clinical and statistical records and confer with other health care professionals on an ongoing basis. Reports regarding statistical findings are required on a monthly, quarterly and annual basis. Continuous quality improvement and evaluation to ensure program delivery is culturally appropriate for diabetes education for clients and staff. Assumes responsibility for own development and maintenance of clinical competence Other duties as required.

REQUIREMENTS



Current registration with the College of Nurses of Ontario as a Registered Nurse Minimum three years' work experience, preferably in a clinical setting Certified Diabetes Educator certification, or willingness to obtain Certified Diabetes Educator (CDE) certification required (800 hours + CDE exam) Use of an automobile, valid driver's license and appropriate insurance are required
Tuberculin (TB) Skin Test, satisfactory Criminal Record Check including Vulnerable Sector Screening Check

Current CPR/First Aid certification is required

Computer skills, with working electronic medical record experience

Superior organizational and time management skills

Effective communication skills- verbal, written and electronic format

Demonstrated ability to work effectively as a team member in a multi-disciplinary environment

Proactive and able to work independently, with minimal supervision

Sound decision making and problem-solving skills

Knowledge and experience working with the Indigenous community

Knowledge and awareness of Indigenous culture and traditions is an asset

Ability to speak the Ojibway and/or Cree languages is an asset
